Compensation Range: $126,750 - $215,480 per year.

Perform initial review and clinical verification of medications/prescriptions, including checking for drug interactions, therapy duplication, and clinical issues based on patient's age, medical conditions, and status.
Conduct final verification on filled medications to ensure correct product usage, absence of damage, intact blister packs, and no missing pills, adhering to quality assurance standards.
Ensure accurate entry of prescriptions and verify correct typing of prescriptions. Accurately perform data entry for various types of prescriptions.
Manage controlled medication inventory, including check-in, documentation, online inventory management, and ensuring appropriate storage.
Utilize Pyxis system and MedBank for prescription verification and controlled medication dispensing (training provided).
Conduct daily checks on emergency kits, ensuring they are properly stocked, and medications are within expiration dates.
Review prescriptions, contacting facilities or prescribers as necessary to address any issues.
Monitor and manage workflow to ensure prescriptions are filled accurately and on time. Prepare compounded medications as needed.
Evaluate prior authorizations and determine if emergency supply should be provided during the review process.
Handle telephone orders from prescribers and delegate prescriptions as required.
Address inquiries from technicians and provide guidance on pharmacy procedures.
Manage electronic communications with staff and facilities, ensuring timely responses.
Maintain a clean and organized workstation.

Qualifications

Computer Skills: Intermediate proficiency with Pharmacy and medication distribution software (Pyxis Medbank, Frameworks, Experience with long-term care pharmacy software preferred.

Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education.
Responsibilities
Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy.
Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy.
Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy.
Serves as a resource of information on medications, pharmacokinetics and interactions, to providers.
Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications.
Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner.
Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse. Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply.
Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid.
Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer.
Maintains confidentiality of all pertinent information to assure that patient and staff rights are protected.
Assist in the identification of potential adverse drug events and in recording such events according to policy and procedures.
Perform literature based research and medication formulary reviews. Assist in development of patient education materials and literature that enhances the clinical pharmacy services provided, in language and at health literacy level of the patient population.
Maintains a clean and organized pharmacy by assuring all pharmacy personal are held accountable for their workspace.
Files complete prescription bundles daily or in a timely manner in an appropriately labeled prescription retention box.

Review emergency CII prescriptions and contact the center or MD to ensure timely receipt of hardcopies.

Job DescriptionStarting at $79,800 MAJOR FUNCTION The Staff Pharmacist reports to and works in collaboration with the Pharmacist-In-Charge (PIC) to serve patients of HJAHC and Henry J. Austin Pharmacy (HJAP) by preparing medications, processing medication renewal requests, providing pharmacological information to the health care team, and monitoring patient drug therapies.
ESSENTIAL FUNCTIONS
Maintains the day-to-day operations of the pharmacy.
Maintains the management of the pharmacy in the absence of the PIC.
Serves in a leadership capacity to oversee the workflow of the pharmacy, including delegation of staff responsibilities.
Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
Oversees management and confidentiality of patients' personal medical data and other personal identifiable information and provides appropriate training to pharmacy staff.
Ensures all services are in compliance with professional standards, state, and federal regulatory requirements.
Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
Maintains pharmacy inventories, including management of virtual inventories.
Reviews prescription orders for completeness, accuracy, and appropriateness.
Enters data, such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge systems, and inventory.
Prepares and dispenses prescribed medications, drugs, and pharmaceuticals for high quality patient care.
Compounds medications using standard formulas and processes such as weighing, measuring, and mixing ingredients.
Directs pharmacy personnel engaged in mixing, packaging, and labeling pharmaceuticals.
Answers healthcare professional and patient drug information requests.
Provides counseling to patients regarding ordered medications, including, but not limited to, indication, directions for administration, potential side effects and interactions, expected time to desired effect, and appropriate storage.
Reviews department of health stock inventory and orders as completed by the pharmacy technicians for accuracy.
Reviews prescription renewal requests and authorizes or denies requests according to the renewal policy.
Maintains pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

ADDITIONAL RESPONSIBILITIES:

REQUIREMENTS:
EDUCATION & EXPERIENCE:
Graduate of an ACPE-accredited school of pharmacy with a B.S. or Pharm.D. degree required
Prior experience working in a community pharmacy and/or ambulatory care setting preferred
Experience working in an urban setting with racially and culturally diverse population preferred
Experience working in a low-income community preferred
LICENSURE AND/OR CERTIFICATIONS:
Licensed pharmacist in the state of New Jersey (or eligible and able to obtain prior to start date)
KNOWLEDGE, SKILLS, ABILITIES AND OTHER (KSAO's)
Bilingual English/ Spanish preferred
