Performs clinical pharmacy services to assigned clinic patients. The pharmacist acts as an integral member of Fred Hutch patient care services, providing pharmacotherapeutic recommendations to providers, nursing, research staff, and other members of the health care team to optimize patient care. Responsibilities include, but are not limited to direct patient care, drug dispensing, research and education, and practice management as it pertains to the care of the assigned patient population. Responsibilities
Clinical:
Collaborate with providers to individualize drug therapies promoting safe, effective and economical patient care.
Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care.
Assume responsibility for monitoring and modifying the drug regimen when referred, to include the ordering and interpretation of laboratory testing.
Provide refill authorization in conjunction with assessment of adherence, disease states, drug interactions, efficacy, and safety.
Act as a drug information resource to providers, nursing staff, research staff and patients.
Ensure patient access to specialty medications by assisting with providing required clinical information to insurers when needed.
Identify patients with indications for drug therapy who are under-treated and optimize drug therapy.
Identify barriers to adherence to medication regimen, incorporating patient characteristics i.e. language, culture/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status.
Utilize tools in educating the patient on self-management of disease and drug therapy.
Review clinic prescribing patterns with clinic staff to design targeted education to promote consistency with UWP guidelines (or other nationally recognized guidelines when needed)
The pharmacist may manage anticoagulation therapy, infectious disease therapy, complementary and alternative therapy, or pain/palliative care for clinic patients under a collaborative drug therapy agreement. Patient care activities include management of all aspects of therapy including initiation, patient education, short term and long term monitoring and management, care transitions, and consults.
Education
Provide primary preceptorship of pharmacy residents and student pharmacists
Provide education to medical/surgical residents, nursing staff and medical students
Present at educational conferences to multidisciplinary staff, as requested
Develop and provide nursing in-services
Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requested
Research
Participate in collaborative research projects with healthcare professionals, pharmacy residents and student pharmacists.
Submit unique work for publication in professional journals
May assist in the preparation, review and implementation of drug study protocols
Collaborating with investigators in support of investigational drug studies
Practice Management:
Implement and maintain departmental policies and procedures related to the safe and effective use of medications
Routinely evaluate current updates in service specific literature
Develop and maintain drug therapy monitoring systems for patients
Establish and maintain efficient lines of communication between pharmacists, physicians, nurses and other members of the health care team.
Participate in pharmacy and hospital committees as requested
Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors
Perform drug utilization evaluation and cost containment analyses, as needed
Dispensing:
Perform duties of a dispensing pharmacist (oral chemo clinic, infusion pharmacy, and/or retail pharmacy) as required
Qualifications
Required:
Graduation from an accredited School of Pharmacy
State of Washington Pharmacist License
Two years of experience in pharmacy practice (months spent in the completion of an American Society of Health-System Pharmacists accredited residency training program may substitute month-for-month for the experience)
Preferred:
Doctor of pharmacy (PharmD) degree, or equivalent, from an accredited School of Pharmacy

WORK SCHEDULE * Full-Time / 40 hours per week * Day shifts HIGHLIGHTS * Be responsible for providing direct patient care (in-person and telemedicine) for abdominal transplant population, including patient education, adherence monitoring, and management of immunosuppression therapy.
* Manage immunosuppression and other chronic conditions (e.g. diabetes, hypertension, antimicrobial prophylaxis) via a collaborative practice agreement.
* Collaborate with the inpatient and ambulatory transplant teams for continuity of care.
* Provide drug therapy consults, participates in treatment protocol development, provides patient/family/staff education, facilitates transition of care, and precepts pharmacy students and residents.
PRIMARY JOB RESPONSIBILITIES
* Ensure appropriate drug therapy including dose, drug-interactions, adverse effects, drug allergies, formulary status, and cost effectiveness.

As a Pharmacist Consultant, you will interact with our clinical and sales teams frequently, building relationships across the enterprise. What you'll do:
Assist pharmacy leadership, sales teams, and producers in analyzing and researching pharmacy cost trends and clinical program opportunities.
Consult external partners, such as large and mid-size employer groups, regarding pharmacy benefits, trends, and strategic direction appropriate for their employee population. Educate account managers, producers, and clients about pharmacy programs and benefits.
Provide clinical drug utilization management, formulary management, and business support to key internal and external stakeholders.
Develop and provide clinical drug information support to stakeholders across the company through writing, researching clinical cases, and standard of care algorithms needed to deliver key pharmacy messages.
Perform drug/medication therapy management services to support the management of drug utilization on the medical benefit, including case and disease management.
Identify and help resolve drug-related issues/problems in concert with medical services staff (medical directors, case managers, and disease managers) to optimize medication therapy and improve clinical outcomes.
Assist case managers in identifying and eliminating barriers to care.
Help design drug therapy components of disease management algorithms.
Recommend appropriate interventions to meet accreditation requirements (NCQA, HEDIS, etc.).
Support the Specialty Pharmacy management program by recommending solutions to improve specialty drug therapy management for high-cost and complex drugs.
Assist in developing and maintaining provider and member communication materials that support clinical pharmacy programs.
Support and serve pharmacy students and/or residents as a preceptor, introducing them to drug utilization management, care management activities, product review, and program development.
