    The pharmacist dispenses prescribed drugs in a healthcare facility environment, transfers prescriptions, provides drug information in collaboration to other healthcare professionals, performs medication order entry into the computer, monitors patient profiles for appropriate medication use oversees the work of the support staff and documents any relevant information. Performs clinical duties including medication therapy management and clinical monitoring with a goal of positive clinical patient outcomes. The position is expect to meet state certifications, requires familiarity with standards concepts, practices, and procedures, and relies on experience and judgement to plan and accomplish goals. * TECHNICAL TASKS * Reviews and interprets patient prescriptions and profiles in order to prevent overdoses, incompatibilities, and toxicity and to ensure the provision of high quality pharmacy services and patient care. Accurately and efficiently processes prescriptions as required for retail and specialty pharmacy patients. * Demonstrates understanding of formulary system and insures compliance with the formulary. * Documents all clinical interventions in the department approved tracking system. * Supervises and reviews all functions performed by support personnel within the pharmacy. * Removes all out-of-date medication from inventory immediately upon discovery and checks all patients on that medication to ensure none will be given. * Maintains physical inventory to insure adequate supply of medications to meet patient demands. * Demonstrates understanding of pharmacy management and clinical programs. * Serves as the pharmacist in charge if needed as required by the Louisiana State Board of Pharmacy * Completes all assigned daily tasks and assists other when time permits. * CRITICAL THINKING * Each Pharmacist is responsible for clinical use and knowledge of Pharmacokinetics and Pharmacological effects of drugs and chemicals on the human body. * Computes calculations appropriate to originate clinical suggestions based on patient parameters such as age, organ function, and weight, to provide more effective usage of targeted medications. * Advises other health care professionals regarding the properties and effects, proper administration, stability and compatibility of drugs to ensure safe and effective patient care. * Utilizes references and computer programs to accurately dose and understand the Pharmacokinetics properties and pharmacology of drugs dispensed by the pharmacy. * Provides in service education to health care professionals and participates when directed in community education activities. * Effectively records and reports data including all interventions that improve patient care, documentation of adverse drug reactions, identification of medication errors, concurrent Drug Utilization Evaluations, and cost-effective therapeutic interchanges. * Able to assess a situation and choose the appropriate course of action. * Demonstrates the ability to communicate effectively with different people taking into consideration any special needs. * INTERPERSONAL RELATIONS * Communicates with physicians and nursing staff/customers concerning the availability, side effects, and possible incompatibilities of new as well as currently used drugs. Demonstrates engagement with pharmacy practices through: reviewing pharmacy newsletter, departmental minutes, and communication with management. * Effectively manages support staff and provides direction in their daily responsibilities. * Expeditiously resolves any communication deficits with other health care professionals and co-workers. * Participates in quality improvement initiatives and follows processes that result from the improvements. * Other Duties As Assigned * Performs other duties as assigned or requested. Experience: Retail experience and working knowledge of 340B and Patient Assistance programs. Education: Doctor of Pharmacy (Pharm.D) License: Licensed as a Registered Pharmacist in MS & RPH
