We are seeking an experienced and motivated Ambulatory Clinical Pharmacy Manager to lead outpatient solid organ transplant pharmacy services. In this vital role, you will oversee clinical program development, guide CDTM protocols, and provide advanced direct patient care to support exceptional outcomes for transplant recipients.
Key Responsibilities
Program Leadership
Lead strategic direction for the Solid Organ Transplant Clinical Pharmacy Program in alignment with institutional and regulatory standards.
Develop, implement, and evaluate clinical pharmacy services for kidney, liver, heart, and/or lung transplant programs.
Maintain clinical guidelines, protocols, and CDTM agreements.
Direct Patient Care
Deliver comprehensive pharmacotherapeutic management for pre- and post-transplant patients.
Optimize medication therapy through monitoring, pharmacokinetic consultation, and individualized treatment plans.
Conduct medication reconciliation, education, and discharge counseling for patients and donors.
Support safe transitions of care across inpatient and outpatient settings.
Collaboration & Education
Partner with transplant providers and care coordinators to integrate pharmacy services into care pathways.
Precept pharmacy residents and APPE students during transplant ambulatory rotations.
Regulatory Compliance
Participate in quality initiatives and ensure compliance with CDTM and regulatory requirements.
Research & Performance Improvement
Contribute to clinical research, quality improvement, and performance metrics related to transplant care.
Present outcomes and support scholarly activities with residents and students.
Stay current with emerging evidence and best practices.
Eligibility Requirements
Education & Licensure
Doctor of Pharmacy (PharmD) from an ACPE-accredited program.
Active NYS Pharmacist License and Registration.
PGY2 Solid Organ Transplant Residency highly preferred.
Board Certification (BCTXP) preferred.
Experience
Leadership or supervisory experience preferred.
Academic medical center or teaching hospital experience desirable.
Research and quality improvement experience preferred.
Skills
Strong expertise in transplant pharmacotherapy.

•Reviewing prescriptions/orders issued by physicians or other authorized prescribers to assure accuracy and appropriate medication management of patients/residents. Promotes use of formulary items by conferring with prescriber as necessary when non-formulary medications are ordered. Processes orders within PIS and assists in maintaining correct patient profiles and charging. Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis
•Monitoring patient profiles to ensure rational drug therapy. Review of drug allergies, drug interactions, and duplicate therapies will be performed during order entry. Reviews and monitors drug therapies that requires dosing adjustments as authorized by the physician. Review labs to ensure medication regimens are appropriate for specific patient/population.
•Performing all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.
•Required to maintain competency for Rome Health's Antimicrobial Stewardship program. Personnel will carry out initiatives set forth by Infection Prevention and P&T Committee. Pharmacists will make monitor, report, and document interventions specifically related to Antimicrobial Stewardship.
•Preparing IV admixtures and extemporaneous parenteral products under aseptic conditions. Ensures that pharmaceutical preparations are sterile, properly packaged, labeled and stored, and not outdated. Ensures compliance with USP up to including assuming the role of an Assigned Trainer. An assigned trainer is assigned by the Designated Person(s) to be responsible and accountable for directly providing the training, observation, and evaluation of personnel for the preparation of compounded sterile and/or non-sterile preparations
•Answering questions and provides information/education to physicians, nurses, students, and patients and their families on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Documentation of such education to patients is to be completed within the medical record when applicable(minimum 25 patient educations annually). Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.

Reporting to the Director of Clinical and Educational Pharmacy Services and supervising two clinical pharmacists, the Clinical Pharmacy Manager is responsible for optimizing pharmacotherapy outcomes through direct patient care, interdisciplinary collaboration, and provider education. Additional responsibilities include evaluating cancer-related medications such as antineoplastic and supportive therapies, contributing to research and quality initiatives, and partnering with revenue cycle teams to expand billable services and strengthen compliance and reimbursement.
Education & Licensure Requirements:
Doctor of Pharmacy (PharmD) degree required
Active license to practice pharmacy in the State of New York

Rome Health is currently searching for a Staff/Clinical Pharmacist.
The Staff/Clinical Pharmacist - Per Diem will ensure safe and effective use of medications through their proper storage, handling, evaluation, preparation, compounding, labeling, dispensing and administration. The Pharmacist must have in depth knowledge of medications and their uses. Duties will include:
•Reviewing prescriptions/orders issued by physicians or other authorized prescribers to assure accuracy and appropriate medication management of patients/residents. Promotes use of formulary items by conferring with prescriber as necessary when non-formulary medications are ordered. Processes orders within PIS and assists in maintaining correct patient profiles and charging. Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis
•Monitoring patient profiles to ensure rational drug therapy. Review of drug allergies, drug interactions, and duplicate therapies will be performed during order entry. Reviews and monitors drug therapies that requires dosing adjustments as authorized by the physician. Review labs to ensure medication regimens are appropriate for specific patient/population.
•Performing all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Supports and maintains a culture of safety and quality.
•Required to maintain competency for Rome Health's Antimicrobial Stewardship program. Personnel will carry out initiatives set forth by Infection Prevention and P&T Committee. Pharmacists will make monitor, report, and document interventions specifically related to Antimicrobial Stewardship.
•Preparing IV admixtures and extemporaneous parenteral products under aseptic conditions. Ensures that pharmaceutical preparations are sterile, properly packaged, labeled and stored, and not outdated. Ensures compliance with USP up to including assuming the role of an Assigned Trainer. An assigned trainer is assigned by the Designated Person(s) to be responsible and accountable for directly providing the training, observation, and evaluation of personnel for the preparation of compounded sterile and/or non-sterile preparations
•Answering questions and provides information/education to physicians, nurses, students, and patients and their families on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Documentation of such education to patients is to be completed within the medical record when applicable(minimum 25 patient educations annually). Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.
EDUCATION,TRAINING,EXPERIENCE,CERTIFICATION AND LICENSURE:
•Graduate of an ACPE accredited school of pharmacy with a BS in pharmacy and/or PharmD degree.
•Current New York State License in good standing and Registration to practice Pharmacy.

Title: Pharmacist - Inpatient
Contract: 13 weeks (with possible extension)
Shifts Available: Days, Evenings, Nights
The Pharmacist Level 1 provides high-quality inpatient pharmacy services by accurately preparing and dispensing medications, supporting the clinical care team with pharmacological expertise, and monitoring patient medication therapies to ensure safe and effective treatment.
Key Responsibilities:
Prepare and dispense medications accurately and in a timely manner.
Answer and resolve questions regarding formulary and non-formulary medications in accordance with hospital policies.
Provide evidence-based dosing and therapeutic recommendations, including age- and kinetics-adjusted dosing.
Perform patient education on medication use, proper dosing, administration times, potential side effects, and drug interactions.
Collaborate with multidisciplinary teams to support optimal medication management.
Maintain compliance with all departmental, state, and federal regulations.
Required Qualifications:
Graduate of an ACPE-accredited College of Pharmacy.
0-2 years of experience as a licensed Pharmacist.
Active Pharmacist license (state-specific requirements apply).
Additional Required Skills:
Chemotherapy preparation (Chemo Prep).
Ability to float to multiple inpatient specialty areas: Critical Care, Pediatrics, Oncology, Transplant, Medicine, and Cardiac units.
Inpatient hospital experience required.
