What you'll do in the role:

Conduct patient visits in diverse settings, including patient homes and various community health center (CHC) locations, to ensure accessible and comprehensive care.
Deliver comprehensive medication management for patients with cardiometabolic and related chronic conditions.
Conduct detailed polypharmacy reviews to identify duplications, drug-drug interactions, adverse effects, cost concerns, and opportunities to simplify therapy.
Collaborate with primary care and specialty providers to coordinate complex care plans and ensure continuity across settings.
Provide clinical pharmacy and patient care services, including medication action, dosing, adverse effects, and drug interactions through a collaborative practice agreement (CPA).
Provide patient and caregiver education, including safe medication use, lifestyle counseling, and adherence support.
Engage in motivational interviewing and shared decision-making to promote patient self-management.
Identify and address care gaps relevant to quality measures (e.g., diabetes control, statin use in ASCVD, hypertension management).
Assess patient health needs to determine whether their prescribed medications are appropriate, effective, and yield optimal patient outcomes.
Regularly review and stay familiar with clinical guidelines and literature necessary to manage patients' care appropriately; Monitor patient progress to determine the effects of each patient's medication on their health.
Identify any untreated health problems that could be improved or resolved with an appropriate treatment plan.
Educate the patient on how to best take their medications while reinforcing the care team's efforts to address other contributing factors, such as recommending supplement utilization, exercise, nutrition, sleep, etc.
Uphold pharmacy compliance with organization policies and procedures (including quality, safety, legal, etc.) to ensure codes and requirements of regulatory agencies are met; Assist with tasks related to Quality Assurance for federal drug discount programs and accreditation for Utilization Review Accreditation Commission (URAC) and Accreditation Commission for Healthcare (ACHC).

Education & Licensure Requirements:

MAJOR DUTIES AND RESPONSIBILITIES:
1. Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.
2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.
3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.
4. Recommends and writes patient-appropriate orders.
5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.
6. Assesses laboratory results and performs pharmacokinetic dosing.
7. Makes appropriate dosing recommendations and documents.
8. Initiates, creates and updates patient medication profiles.
9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.
10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.
11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.
12. Accurately submits electronic claims for medications, as applicable.
13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.
14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.
15. Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.
16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP standards and company policies and procedures by all pharmacy staff.
17. Ensures required documentation is completed on QA/QC records and compounding documents.
18. Completes company-required orientation and annual competencies and in services.
19. Provides concise, applicable and timely responses to requests for drug information.
20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates
21. Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient's need.
22. Plans work to ensure cost-effective delivery methods are utilized.
23. Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.
24. Delegates non-clinical tasks to assigned support personnel.
25. Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.
26. Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.
27. Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.
28. May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.
29. Performs other related duties as directed by supervisor.
