What will I do?
Utilize and bring your coursework to life, while gaining a comprehensive knowledge of the retail pharmacy industry
Work with your preceptor, Pharmacy Manager, pharmacists, and technicians to learn to better serve customer needs
Answer questions, provide counseling on prescriptions, and relay any pertinent information so customers can successfully and confidently use medication at home
Proactively contact internal and external business partners; comply with all federal, state, and HIPAA regulations; ensure company guidelines, standards, policies, and procedures are followed
Required Qualifications:
In progress towards earning Pharm D degree in an accredited School of Pharmacy
Pharmacist Intern Permit where applicable by state law

JOB REQUIREMENTS
EDUCATION:
Work requires at a minimum a BS Degree in Pharmacy from an accredited college of pharmacy or doctor of pharmacy. A master's degree in related field (MBA, MPH, MS in pharmacy) is preferred but not required. Completion of specialized clinical residency in Hematology Oncology or other specialty settings preferred.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Work requires licensure to practice pharmacy in the Commonwealth of Massachusetts by the hire date.
Board Certified Oncology Pharmacist (BCOP).

EXPERIENCE:

* Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications.

* Provides drug information to health care professionals and to patients.
* Documents clinical interventions.
* Counsels patients on discharge medications as appropriate.
* Performs clinical review and verification of medication orders.
* Performs medication order entry when needed.
* Discusses medication order clarifications with the prescriber, documents interventions, and communicates medication order changes as needed.
* Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy.
Operations/Distribution Performance
* Ensures the timely and accurate dispensing of medications from the pharmacy area.
* Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, non-sterile and sterile compounding, chemotherapeutic, hazardous, and investigational agents.
* Responsible for the accurate filling and dispensing of medications from the pharmacy area.
* Cross-covers in a variety of practice areas within the pharmacy as requested by supervisor.
* Ensures regulatory compliance of controlled substance distribution and monitoring.
* Supervise work of pharmacy technicians.
* Provide feedback to maintain and optimize pharmacy technology.
Financial Performance
* Promotes the use of formulary drugs by converting non-formulary orders to formulary drugs when possible according to P&T protocol. Coordinates procurement of non-formulary drugs when necessary.
* Conducts targeted drug program and drug usage evaluations as needed and reports results to Department Managers/ Pharmacy and Therapeutics Committee.
* Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens, drugs, and by reducing waste.
* Facilitates cost saving programs such as IV to PO and therapeutic substitution.
* Manages medication inventory, expiring products, and recalls.
Safety and Regulatory Performance
* Detects, monitors, documents, and reports adverse drug reactions and medication errors.
* Utilizes drug use reports to identify adverse drug events.
* Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.
Research and Education Performance

Job Summary

Collaborate with Legal and Benefits to ensure the Evidence of Coverage language meets the needs of the Pharmacy program.

1. Directs and coordinates the clinical activities of the assigned pharmacy service area. Responsible for preparing, compounding and dispensing medications to patients in accordance within state, federal laws.
2. Reviews and accurately interprets physician's orders checking for appropriateness of orders as to indication, clarity, accuracy of dose, drug allergy, potential drug interactions and duplication (two or more drugs with the same therapeutic action).
3. Contacts and confers with physicians if there are any questions or irregularities regarding medication orders. Interventions with outcome are routinely in the Meditech Clinical Intervention routine.
4. Ensures that medication is accurately prepared and labeled with all necessary and appropriate information. Supervises support staff (when applicable) during an assigned shift. Ensures the pharmacy is operated within the limits of State and Federal regulations.
5. Provides accurate, appropriate and timely drug information utilizing knowledge of pharmacology, reference books and scientific literature.
6. May conduct medication history interviews, reconcile patients' medication therapies and provide drug information to patients and their families.
7. Accurately anticipates controlled drug needs of each patient care area based on completed Controlled Drug Disposition Record.
8. Completes medication station inspections are completed by designated Pharmacy Assistants (or in the absence of a Pharmacy Assistant by the Pharmacist).
9. Participates in medication education by discussing medications with the patient and appropriately documenting in the patient's progress notes. Understands and participates in the Medication Use Evaluation.
10. Must be able to perform order entry and utilize all Registered Pharmacist functions of the Pharmacy computer system accurately and efficiently. All necessary information is entered including drug allergy information, information for pharmacists and nurses and information necessary for accurate charging.
11. Participates in multidisciplinary team meetings, review discharge medication lists from skilled nursing facilities and inpatient hospitalizations, improve medication reconciliation process, and provide medication optimization consults for patients.
12. May conduct home visits for the identified patient population to review medications, help the patient understand how to take medications as prescribed and provide education such as inhaler teaching to optimize medication therapies.
13. Participates in Pharmacy Quality Programs that relate to: People; Clinical excellence; Operational excellence; Financial performance and growth; Safety and quality; and Research.
14. Collaborates with other pharmacists to provide operational support. This includes weekend, holiday, evening, night and on-call coverage as described in departmental policies and procedures.
15. Provides continuing education for the clinical staff (e.g., pharmacists, physicians, nurses), serves as preceptor for pharmacy student, interns, and residents.
16. May participate in the development and implementation of treatment guidelines, protocols, and clinical pathways.

Responsibilities
* Practices as a pharmacist in accordance with all federal, state, DFCI and Joint Commission policies, regulations, and standards.
* Ensures medication orders and/or prescriptions for appropriateness, accuracy and completeness based on patient-specific parameters and clinical data.
* Provides clinical consultation and evidenced based recommendations for treatment plans, supportive care, dose adjustments and monitoring plans to clinicians regarding pharmacotherapy needs for patients based on literature, clinical practice guidelines and institutional standards.
* Collaborates as a member of the pharmacy and interdisciplinary team through patient care related meetings including rounds, huddles, and case conferences as applicable.
* Provides comprehensive education to patients and caregivers receiving oncology related therapy including participation in the oral anticancer education program and review and maintenance of patient education materials.
* Documents clinical care activities in the EHR and effectively communicate all changes in care plan to the staff involved in patient care.
* Participates in scholarly and educational activities according to departmental expectations (e.g., in-services, drug use evaluation, resident precepting, clinical research, quality/process improvement, presents continuing education programs, guideline development).
* Participates in clinical administrative duties such as pathways, beacon validation and active patient management, formulary request process and presentation to P&T committee(s), evaluate off label/exception review, REMS programs and other departmental or strategic initiatives.
* Participates in the development of policies, procedures, teaching documents, education and/or monitoring tools, workflow design, and other initiatives that will improve and/or expand the program.
* Other duties and projects as assigned.
Qualifications
The Clinical Pharmacy Specialist, Melanoma and Cutaneous Oncology must meet the following requirements to be hired and maintain their position in the Pharmacy Department at DFCI.
Minimum Education:All Pharmacists require a Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy.
Minimum Experience:PGY2 residency in oncology (preferred) or PGY1 plus 2 years of clinical specialist experience in oncology or 5 years of clinical specialist experience in oncology required.
License/Certification/Registration:Must be licensed to practice pharmacy in the Commonwealth of Massachusetts. Pharmacists must attain board certification in their specialty area within 18 months of employment.

* Ability to resolve common problems within their scope of practice and to escalate appropriately when needed.

+ As an RPh Advisor you will be directly supporting Medicare Part D members and providers with requests related to their pharmacy benefits. You will be responsible for ensuring cases are accurately set up for our members, review clinical information for decisioning the request, performing outreach to providers for more information when needed, and ensuring accurate decisions that comply with compendia and Medicare guidance and timelines.
