Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record; and checking of IV products. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, clinical protocols per pharmacy and pharmacist interventions).

Qualifications
We are seeking an exceptionally-skilled and compassionate pharmacist with the following qualifications:
+ Doctor of Pharmacy Degree
+ California Pharmacy license
+ ASHP-accredited PGY2 oncology residency or at least a minimum of 1 year of pharmacy practice experience in oncology
+ Recent clinical oncology experience (within 5 years)

Position Summary: The ClinicalPharmacist works with patients with chronic conditions and the primary care clinician team, to ensure optimal results from drug therapy for these chronic conditions, and to support patients in taking more of an active role in medication and healthcare self-management; as well as to assist clinicians in providing individualized patient care for patients.
Duties and Responsibilities:
Manage medication therapy for patients served at SCC including but not limited to Diabetic, Hypertension, Hyperlipidemia, in collaboration with patients' primary care providers and following SCC protocols.
Review medical records in OCHIN-EPIC and other systems (Pharmacy software) as needed; Identify qualifying and co-morbid chronic medical conditions.
Evaluate treatment regimen and identify potential drug related problems.
Adjust treatment plan based on identified potential drug related problems, and medical conditions.
Assess and monitor the patient's knowledge of medications/disease, patient's perceived medication-management issues, medication adherence, adverse effects, drug interactions and lifestyle modifications.
Order appropriate laboratory test(s) related to medication therapy.
Initiate/ adjust refill/ discontinue medication(s) per established protocols.
Monitor for adverse effects, complications, and therapeutic outcomes.
Provide and reinforce patient education
Respond to medication refill requests following SCC Medication refill protocol Clinical.
Review and evaluate treatment plans by Saban Community Clinic providers at all 4 clinic sites and make recommendations to providers when appropriate.
Sort, review, and respond to requests for medication refills as per protocol approved by SCC,
assist with approving alternate medication requests for ones that are not covered by insurance, assist with prior authorizations, and requests for new medications.
Approve, adjust dosing, deny refill requests according to departmental protocols approved by Saban providers.
Suggest formulary substitutions for medications.
Order appropriate laboratory tests as indicated.
Troubleshoots transmission errors in e-prescribing/e-Scripts.
Assist in coordinating patient appointments when applicable to medication refills.
Responsible for serving patients of the SCC pharmacist when coverage is needed
Accurately filling prescription orders and managing the dispensing function, consulting with patients and prescribers about medication regimens, providing pharmacist care and performing patient consultations on drug therapies.
Provide appropriate support to pharmacy technicians including directing technicians' workflow and verifying their order entries, preparations and labeling of pharmaceuticals.
Actively participate in pharmacy purchases and inventory management in compliance with CA Pharmacy Laws.
Ensure accurate electronic billing of medications dispensed to insured patients, according to PBM contracts.
Will be required to precept pharmacy students and residents through Saban Community Clinic's collaboration with pharmacy schools and residency programs.
Position Requirements:
A Doctor of Pharmacy (Pharm.D.) degree (Required)
PGY1(Community or Ambulatory) residency training or equivalent clinical experience (One to two years prior experience in a similar position providing Medication Therapy Management in a community clinic setting, particularly working with patients with Diabetes, hypertension, and hyperlipidemia (Required)
California Advanced Practice Pharmacist License, and or board certification (Highly Preferred)
Bilingual in Spanish preferred.

POSITION SUMMARY:

Insures compliance with all policies regarding security, patient safety, equipment maintenance and patient confidentiality according to California State Board of Pharmacy, DEA and other regulatory agencies.
Knowledgeable and competent about HRSA 340B program and ensures compliance with 340B statue and regulations.
Keeps working area in a clean and orderly manner.

Practices Pharmacy in a community setting with clinical functions. Responsible for day-to-day pharmacist duties.
