Salary Range: $103.39 - $127.88 Summary of Duties: Responsible for reviewing physician medication orders (EPIC CPOE) for appropriateness cost-effectiveness, monitor and adjust patient drug therapies according to many pharmacy driven protocols, and provide drug information to patients and other healthcare providers. Providing pharmaceutical care including anticoagulant, PK, renal dosing, IV to PO, insulin dosing, antimicrobial stewardship, and many other pharmacy protocols.
