The ClinicalPharmacist implements Monogram's medication therapy management (MTM) program, ensuring appropriate selection and use of medications by our patients. As a 1099 contractor, the ClinicalPharmacist will perform timely written comprehensive medication reviews (CMRs) by utilizing patient-provided medication lists, prescription claims, and available clinical information (lab values, etc.). The pharmacist will create drug therapy action plans based on findings and medication recommendations when appropriate for Monogram Health patients, then communicate these recommendations to providers and clinical staff at Monogram Health and in the community. The pharmacist will conduct telephonic consultations with medical providers when medication changes appear necessary and speak with patients about medication use concerns. The pharmacist will work collaboratively with their local Market Physician Executive (POD physician) and clinical staff (e.g., Advance Practice Providers (APPs), nurses, and social workers) to integrate medication management into Monogram Health's overall care model and to ensure that field clinicians are deploying practices to optimize patient medication use and outcomes.
Roles and Responsibilities
Perform and write concise patient-specific comprehensive medication reviews (CMRs) consistent with Monogram productivity and quality standards.
Drive meaningful medication interventions that result in medical providers' and patients' adherence to evidence-based medication guidelines.
Review patient care reports, clinical notes, and other medical records as needed for drug therapy action plan development, targeted clinical outreach programs, and quality assurance purposes.

Partner with IV room pharmacists to ensure adherence to USP standards, media fill testing, and competency requirements.
* Routinely report to the Director of Pharmacy (DOP) on staff compliance with sterile and nonsterile compounding processes, ensuring adherence to USP standards, patient safety requirements, and all applicable regulatory guidelines.
Collaborate with the Investigational Drug Service when needed to assist in preparation and accountability of investigational medications.
* Provide staffing coverage, work every 4th weekend evening, and on-call as needed, supporting key functions such as order entry, IV room operations, and participation in departmental projects (e.g., drug use evaluations [DUE]) and initiatives aligned with patient care goals.
* Routinely staff minimum of 3 evening shifts per week, works every 4th weekend (evening shifts), 1 summer holiday, 1 winter holiday
RECRUITMENT REQUIREMENTS

CLINICALPHARMACIST SPECIALIST, AMBULATORY ONCOLOGY (FT; 40hrs/wk) - Temple Health/Fox Chase Cancer Center - (254250) Description The ClinicalPharmacist Specialist-Ambulatory Oncology delivers comprehensive pharmaceutical care to cancer patients across all treatment phases- in addition to clinical interface with physicians and nurses, optimizing medication therapy for cancer patients, and collaborating with members of the multi-disciplinary team to ensure provision of safe, effective, and cost-efficient care.
This role supports clinical, operational, and educational initiatives to enhance patient outcomes, safety, and quality of life.
Serves as a resource for complex oncology-related drug therapy decisions, contributes to formulary guideline development, and provides training to healthcare professionals.
Responsibilities include: serves as the primary oncology pharmacist liaison in infusion centers and clinics; providing direct, patient-facing education and clinical care; supports oncology formulary management efforts, including evaluation of new medications, therapeutic interchanges, and strategies for optimizing safety, efficacy, and financial performance; develops and maintains oncology-specific guidelines, policies, and protocols to enhance safety and clinical effectiveness; Serves on Oncology and Infusion Center Committees.

Summary
Provides quality pharmaceutical care, and fills/dispenses medications in accordance with the latest pharmacy standard practices. Reviews prescriptions and medication orders for appropriateness and compatibility with current therapy, disease states, drug allergies, adverse drug effects, duplication of therapy and compliance to ensure patients' medication therapy is safe, effective, timely, and free from drug-related problems. Job Duties
Prepares and dispenses medications for infant, pediatric, adolescent, adult, and geriatric patients.
Evaluates evidence-based literature to offer appropriate clinical recommendations in order to optimize drug selection and individualization of therapy.
Ensures and executes policies and procedures that ensure appropriate, cost-effective, and safe medication use.
Performs clinical medication surveillance, documents interventions, reports adverse drug reactions and medication errors.
Coordinates the activities of pharmacy technicians engaged in the preparation, labeling, and delivery of medications and other pharmaceutical supplies.
Counsels/Provides drug information pertaining to medications, drug usage, and compatibility to physicians, nurses, other healthcare professionals, and patients in accordance with OBRA or other regulatory standards.
