Job Summary
What You'll Do
Deliver comprehensive medication management for patients with chronic conditions such as diabetes, hypertension, and heart failure.
Practice under Collaborative Drug Therapy Management (CDTM) agreements to initiate, adjust, or discontinue medications.
Provide medication reconciliation and transitional care support for patients discharged from the hospital or emergency department.
Offer individualized patient education and counseling to improve adherence, safety, and therapeutic outcomes.
Collaborate closely with primary care teams to develop and document care plans aligned with clinical guidelines.
Identify and address medication-related problems including polypharmacy, affordability concerns, and drug interactions.
Support practice and system-level quality and performance improvement efforts related to medication use and chronic disease outcomes.
Document clinical activities in the Epic EMR and participate in data collection for program evaluation.

Responsibilities:
Reviews prescriptions issued by physicians / authorized prescribers to assure accuracy and determine formulas and ingredients needed for dispensing the order.
Collaboratively assesses the suitability of each patient for home infusion therapy.
Assists the referral source in developing and implementing a plan for each patient's smooth transition from hospital to home.
Develops, implements, and monitors patient's individualized plan of care to include setting of therapeutic goals, monitoring of progress to goals and ongoing communication with prescribing physician.
Acts as a clinical resource for all members of the healthcare team on the appropriate use of medications, possible drug interactions, medication side effects, dosage, and storage information.
Provides written education materials and verbal consultation, as needed, to patients and / or caregivers relative to the medication prescribed for home infusion.
Demonstrates competency in the preparation / compounding / dispensing of medications according to NELC policies and procedures.
Monitors all physician orders for drug/drug interactions, duplication of therapy and appropriateness of drug order. Makes recommendations for change(s) to physician in a timely manner.
Detects and evaluates the clinical importance of adverse drug reactions and complications associated with home infusion therapy informing the physician as required.
Comprehensively documents patient's progress to goals of therapy and all communication re: patient status and changes in therapy according to JCAHO documentation standards.
Effectively communicates patient requirements and changes in patient status to appropriate members of infusion and reimbursement teams as well as the HHA staff involved in meeting the patient's home health needs.
Demonstrates competency in the use and management of medication administration equipment.
Ensures that confidentiality of patient information is maintained in accordance with state and HIPAA Privacy and Security regulations.
Educational / Professional Requirements:
Bachelor of Science degree from an accredited school of pharmacy.
Registered Pharmacist, in good standing, with a valid license.
Skills:
Excellent interpersonal skills.
Excellent organizational skills.
Ability to apply mathematical operations to such tasks as dosage calculation, formulation of pharmaceutical preparations and data analysis.

Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.
Job Summary
The Clinical Pharmacy Manager is responsible for overseeing the clinical programs of the pharmacy department. This position will utilize Cardinal Health resources to provide safe and effective clinical pharmacy programs at the hospital. This position will assist the Director of Pharmacy to assure all legal, regulatory, and accrediting requirements are maintained at the best demonstrated practice. Assure clinical pharmacy services and activities are carried out to meet goals and standards of both Cardinal Health and the client/hospital. Responsibilities
* Provide clinical support and direction to the Director of Pharmacy (DOP) and pharmacy staff in the hospital regarding the implementation and operation of clinical pharmacy programs. This includes: direct discussions and dialogue with all health care professionals in the hospital; providing pharmaceutical care to selected patients; and developing drug therapy review programs and documenting their results.
* Assist with the leadership of the hospital P&T committee, assuring (but not limited to) the following items are part of the committee meetings: current drug trends, formulary additions, formulary deletions, annual formulary approval, non-formulary drug use, drug shortages, cost control initiatives, updates to the Policy and Procedure manual, annual review of order sets and protocols, black box warning, ADR reporting, pharmacy intervention reporting, controlled substance audit reporting, automation reporting.
* Actively participates and when required helps to develop an Antimicrobial Stewardship program in conjunction with multidisciplinary team members. Develop and implement treatment guidelines, protocols, formulary changes and critical pathways as approved and authorized by appropriate committees.
* Participate in hospital committee meetings necessary to enhance practice and patient care.
* Assists in meeting or exceeding all Accreditation Bodies (TJC, DNV, CMS, BOP) standards related to pharmacy.
* Participates in the development and maintenance of information systems and other technology applications within the Pharmacy.
* Use Clinical best practices in formulary and cost control.
* Use Cardinal Health applications like DCOA, to analyze data and manage clinical initiatives.
* Facilitate the identification, resolution, implementation, and communication of cost-effective drug use issues in collaboration with the medical staff.
* Document activities in accordance with Clinical Monthly Report requirements.

* Maintains all required training and education per department requirements.

* Direct and coordinate clinical clerkship programs for pharmacy and other health care professional trainees.
