Job Description:
The Pharmacist is responsible for reviewing, interpreting, and verifying prescriptions to ensure accuracy and safety. This role involves providing guidance to pharmacy staff, ensuring compliance with quality and regulatory standards, assisting patients with medication-related questions, and maintaining accurate pharmacy records and inventory. The Pharmacist will also support retail pharmacy operations by multitasking in a fast-paced environment, communicating effectively with patients, and ensuring high-quality service delivery.
Responsibilities:
Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals. Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage or pharmaceuticals.
Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hClientardous waste drugs.
Enters data such as patient name, prescribed medication and cost, to maintain pharmacy files, charge system, and inventory.
Assays medications to determine identity, purity, and strength. Instructs interns and other medical personnel on matters pertaining to pharmacy.
Experience:
Minimum of 1 year retail pharmacy experience required (CVS, Walgreens, Target, Walmart, Rite Aid, or similar).
Experience counseling patients on medications and managing high-volume prescription workflows.
Experience entering and verifying prescription data in pharmacy computer systems.
Skills:
MUST be Licensed in IL
Retail Pharmacy experience (i.e. CVS, Walgreens, Target etc)
Prefer at least one year history and prefer it to be with a retail organization i.e. Walgreens/CVS

Basic Qualifications

* Ensure that medications reach the patient in the correct dose, form, vehicle and at the correct time. This includes tablets, liquids, drops, capsules, ointments, injections, inhalers, creams, beads, liposomes, continuous delivery systems and other forms that medications may take.
* Take responsibility for the accurate, appropriate and timely use of medications administered to patients by staff in the clinic setting.
* Take responsibility for the accurate, appropriate and timely dispensing of medications for patients upon presentation of valid prescriptions in the outpatient pharmacy setting.
* Supervise and monitor the work of pharmacy technicians in compliance with State and Federal Requirements.
* Teach and respond to queries of patients and their family's providers and other staff regarding accurate, appropriate and timely use of medications including indications, dosing regimens and side effects.
* Stay current on new information and developments and educational material in Pharmacy to serve patients, providers and other staff.
* Collaborate with providers and other health care staff including Community Service, Behavioral Health, Primary Care and other Medical Specialties to deliver safe, effective and economic medication use and delivery.

Processes all medication order information into the Electronic Health Record to ensure accuracy of records regarding past, current and future dispensing and charging.
Maintains a comprehensive pharmacy patient profile. The profile should include age, maturation, weight, allergies, diagnosis, estimated and actual pharmacokinetic parameters, and pertinent information which may affect medication therapy decisions.
Oversees the packaging labeling, and distribution of medications for all patients and employees according to established policies, procedures, and guidelines. Monitors the use of scheduled medications on the nursing units and maintains controlled inventories of medication in the dispensing areas.
Compounds and dispenses pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products.
Verifies medication orders before delivery to patient care areas and ensures accuracy and completeness. Resolves discrepancies as necessary.
Reviews all laboratory data for appropriateness of medication use and recommends changes when needed. Including microbial culture and sensitivity data, toxicology, therapeutic drug monitoring, etc.
Develops and presents orientation, education and training programs for medical, nursing, hospital and pharmacy staff, as well as assisting in the educating of pharmacy students, residents, and fellows.
Educates patients and their families about specific medications.
Responds to emergency situations to assist with medication management (e.g. response to cardiac arrests)
Provides accurate and timely medication information and or teaching to healthcare professionals or patients.
Maintains the hospital formulary and acts to limit medication usage to formulary items when appropriate
Documents all clinical activities and interventions performed. Collects data, maintains logs, records and other documentation as required by the department
Communicates professional knowledge effectively in both oral and written forms to patients and other health professionals.
Participates in quality improvement and medication use review activities of the department.
