APPE Summary: Pharmacists participate in pharmacy-run clinics: Anticoagulation, Hepatitis C, and Spirometry. Each year 5-10 senior pharmacy student's complete clerkship rotations at our site. Drug information resources and computer support are readily available. Rotation experiences will vary to student preferences. Key Responsibilities Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems. Provide targeted medication interventions (TMEs) and follow-up to support adherence and safety. Collaborate with prescribers, health plans, and care teams to resolve therapy issues. Document interventions clearly and maintain compliance with MTM program requirements. Educate patients on safe, effective use of their medications and address barriers to adherence. What You'll Bring Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree. Licensure: Active and unrestricted pharmacist license in the U.S. Experience: Retail, hospital, ambulatory care, or prior MTM experience is a plus-but all licensed pharmacists are encouraged to apply, including new grads.
