Position Summary
The role of the ClinicalPharmacist reports to the Director of Pharmacy. This position is responsible for reviewing medication orders, fills orders for medications, and monitors patient drug therapies. This position provides drug information to interdisciplinary team members patients, as required. This position will oversee day-to-day operations of the department including overseeing support personnel. This position will serve as back up to the director, as required.
Essential Position Responsibilities
* Review medication orders to ensure accuracy
* Monitor patient drug regimens and partner with interdisciplinary team members to optimize care
* Supervise and provide guidance to pharmacy support personnel and students
* Perform medication history with patient, educate patient on proper use of medication and potential adverse effects, and prepare and dispense pharmaceuticals to patients
* Participate in multidisciplinary committee meetings, as assigned
* Ensure records maintained as required by state and federal law
Qualifications
* Education: Must be a graduate of an accredited college of pharmacy
* Licensure and Certifications: Licensed as a pharmacist in the state of Arkansas
* Experience: Previous experience in an acute care pharmacy setting, preferred.
Work Environment: This position will spend 80% of time sitting while performing work in a standard office environment and 20% of time standing and/or walking while pushing, pulling, lifting and/or carrying up to 50 lbs.

Healix Infusion Therapy is seeking a PRN ClinicalPharmacist for one of our Infectious Disease Office Infusion Centers located in Fayetteville, AR!
Our Clinical Staff Pharmacist oversee onsite infusion pharmacies and are responsible for the compounding, dispensing, and monitoring of medications for patient use at home or in the infusion center. The Pharmacist conduct clinical consultations frequently with physicians regarding drug therapy and interact daily with the office infusion center nursing staff to coordinate patients' medication therapy plans.
SCHEDULE:
Standard day time clinic operating hours. Looking for someone with weekday availability to pick up shifts.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Ensures that drugs and/or devices are dispensed and delivered safely and accurately. This is accomplished by fulfilling and dispensing orders appropriately, identifying, resolving and preventing potential and actual drug-related problems, and optimizing patient therapy outcomes. Directs and coordinates pharmacy and site operations as requested.

Ensures that the pharmacy meets all state and federal laws and regulations, while working in the pharmacy.
Promptly and accurately receives, interprets, evaluates, prepares, and dispenses prescription drug orders.
Monitors therapy for each patient, perform pharmacokinetic monitoring and maintains appropriate drug use for each patient and client. This is evidenced by audit scores on pharmacy assessments, care plans and progress notes, medication profile reviews and updates, and therapeutic outcomes.
Provides to the patient or patient's agent accurate and meaningful verbal and written drug and/or device information.
Monitors therapy for each patient, performs pharmacokinetic monitoring and maintains appropriate drug use for each patient and client.
Responsible to maintain required production and dispensing duties accurately as the assigned shift requires.
Maintains appropriate oversight of the intravenous compounding and checks all pharmacist-required compounds prior to mixing.
Insures that the clean room is maintained according to policy and law and regulation.
Complies with Healix Policies and procedures and recommends modifications as necessary.
Arrives at site promptly and is present for all assigned shifts. Notifies manager and Pharmacy Staffing as soon as possible for any anticipated outages.
Selects cost-effective and clinically appropriate drug products, methods of delivery, and delivery systems as permitted by prescribing physician.
Develops, implements, and/or enhances pharmacy care programs and/or services that enhance and add value to the client's practice.
Maintains optimal drug and supply inventory and reports are submitted in a timely basis.
Insures that contracted drugs are ordered, received, stored, and secured properly.
Insures that adequate security to prevent indiscriminate or unauthorized access to pharmacy and confidential patient records are maintained.
Communicates effectively with client.
Participates in pharmacy staff meetings on a regular basis.

Responds to pages while on-call within 15 minutes.

2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.
3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.
4. Recommends and writes patient-appropriate orders.
5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.
6. Assesses laboratory results and performs pharmacokinetic dosing.
7. Makes appropriate dosing recommendations and documents.
8. Initiates, creates and updates patient medication profiles.
9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.

11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.

Position Summary

2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.
3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.
4. Recommends and writes patient-appropriate orders.
5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.
6. Assesses laboratory results and performs pharmacokinetic dosing.
7. Makes appropriate dosing recommendations and documents.
8. Initiates, creates and updates patient medication profiles.
9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.
10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.
11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.
12. Accurately submits electronic claims for medications, as applicable.
13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.
14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.
15. Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.
16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP standards and company policies and procedures by all pharmacy staff.
17. Ensures required documentation is completed on QA/QC records and compounding documents.
18. Completes company-required orientation and annual competencies and in services.
19. Provides concise, applicable and timely responses to requests for drug information.
20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates
