Job Description:
Educate and counsel patients, families, and healthcare team members on safe and effective medication use. Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs. Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up. Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development. Identify, report, and help resolve medication safety events, adverse drug reactions, and errors. Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation. Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity. Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas. Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments. Other duties as assigned. Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).

Job Description
SUMMARY OF POSITION:
The staff pharmacist works according to legal and ethical guidelines to ensure the correct and safe supply of medical products to Axium Healthcare Pharmacy patients. They are also involved in maintaining and improving patient health by providing pharmaceutical care services including, but not limited to: oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. Essential Duties and Responsibilities:
* Prepare, label and dispense medications for patients in accordance with
physicians' prescriptions; ensure compliance with Federal, State and local laws
and regulations pertaining to the dispensing of drugs and narcotics and the
maintenance or required records.
* Maintain appropriate records of dispensing and oversee and maintain the systems
that charge or credit for medications and medication treatments.
* Provide drug information and educate caregivers, patients and students
regarding medications and their proper use and monitoring.
* Oversee and monitor all aspects of the medication process; collect data and
present reports and recommendations regarding issues with the medication
systems.
* Ensure proper documentation and record keeping is done for all medications with
particular attention to controlled substances.
*Oversee all equipment and systems used in preparation, oversight and dispensing
of medications.
* Ensure that appropriate medications are available for patients and properly procured,

* Verify billing accuracy for accountable reporting, such as the Marion County programs.

The contracted Relief Pharmacist is responsible for the smooth and successful operation of the pharmacy and the maintenance of all business-related functions at his/her respective Center while on duty, including but not limited to, medication therapy management, central fill, and/or community pharmacy operations. He/She is responsible to compound and dispense prescribed medications, drugs, and other pharmaceuticals for patient care by performing duties in both the adjudication and production work environments while providing direct supervision of Pharmacy Staff, Pharmacy Services Representatives, Pharmacy Technicians, and Pharmacy Support Specialists performing pharmacy related duties.

Develop and maintain a good working relationship with physicians and other health care professionals to better assist the patients
Compound medications, using standard formulas and processes such as weighing, measuring, and mixing ingredients, if applicable
Direct pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
Actively participate in training and developing new team members on accurate completion of job tasks while on duty
Provide mandated counseling on all new and changed therapies on both prescription medications/items and over the counter medications/items in accordance with True Health's policy and as required by state and Federal law
Answer questions and provide information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals as needed

Maintains a clean, neat, and orderly pharmacy department at all times while on duty
