Ensures appropriate, cost-effective drug utilization and pharmaceutical care while maintaining and enhancing patient care as an interdisciplinary team member in support of patients, providers, and ancillary staff. Provides members with continuous medication treatment where appropriate per authorization or utilizing authorized protocols supplied by software vendors built in the electronic medical record or authorized KPHI clinical pharmacy protocols. Essential Responsibilities: * Provides pharmaceutical care services for defined high risk patient populations. * Ensures continuity of pharmaceutical care and optimal utilization of resources to and from acute and ambulatory patient-care settings. * Recommends and/or reviews laboratory tests to assess patient response to drug therapy. * Identifies medication therapy problems as appropriate. * Analyzes requestors drug information need. * Employs systematic, efficient, and thorough procedure for retrieving drug information. * Follows up consistently on drug information recommendations and assesses effectiveness. * Performs drug regimen reviews for members as warranted. * Assesses patients where necessary and communicates information to physician. * Provides direct oversight for functions and activities of support personnel to assure responsibilities are performed in accordance with scope of practice and applicable procedures. * Adapts effective strategies for communicating with non-English speaking patients or those who are otherwise impaired (e.g., blind, deaf, cognitively impaired, illiterate). * Designs and recommends pharmacotherapeutic regimens and corresponding monitoring plans to prescribers in way that is systematic, logical, and secures consensus from prescriber and patient. * Monitors data and modifies pharmacotherapeutic plans as necessary. * Documents pharmaceutical care activities in patients' medical records or clinical information system. * Participates, as appropriate, in development and/or implementation of regional drug initiatives or other formulary management activities. * Participates in establishing disease state management protocols in coordination with HPMG physicians, Clinic Nursing, and the Pharmacy and Therapeutics committee. * Collaborates with inpatient/outpatient pharmacist or established Regional disease management program(s) for follow-up with high-risk discharges. * As needed, participates in designing, reviewing, monitoring, or researching clinical drug trials. * Designs effective medication-use education for patients and caregivers. * Provides one-on-one counseling to patients and caregivers, including information on drug therapy, adverse effects, compliance, appropriate use, and handling. * Provides patient-specific verbal or written consultations, including pharmacotherapy and pharmacokinetic, to healthcare practitioners. * May perform limited physical examinations, such as blood pressure and glucose measurement, for the purpose of providing medication management services. * Participates in monitoring and improving relevant outcome measures and/or quality improvement (QI) indicators. * Defines QI processes. * Develops measures to monitor, assures safety and appropriateness. * Participates in Regional medication management and formulary education programs to promote appropriate, cost-effective prescribing. * Participates in task forces, lecture programs and skills certification for patients, providers, pharmacy staff, and support staff at departmental, Regional, national, and international levels as warranted. * Establishes relationships with physicians and facility leaders to successfully partner on drug therapy issues. * Balances cost and quality issues. * Participates in didactic or clerkship education of pharmacy students, and/or serves as preceptor of residents as warranted.

RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES: - Implements, maintains and provides clinical pharmacist services including: clinical, educational, informational and distributional functions for newborn, infant, pediatric, adolescent, adult and geriatric patients of The Queen's Medical Center (QMC). TYPICAL PHYSICAL DEMANDS: - Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking. - Frequent: pushing/pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object. - Continuous: repetitive arm/hand motions. - Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling up to maximum 50 pounds. - Operate pagers, computers, printers, typewriters, telephones, fax machines, dispensers, laminar flow hoods, TPN compounding machines and other pharmaceutical and office equipment. III. TYPICAL WORKING CONDITIONS: - Not substantially subject to adverse environmental conditions. - Subjected to noise/vibration from air conditioner, printers, time stamp machines, pneumatic tubes, telephone, etc. - Exposure to body fluids, chemicals, some contact with patients with communicable diseases. - May be required to work with hazardous drugs, depending upon area of assignment. - Work environment is fast paced, congested and may be stressful due to workload. - Interacts with patients, nurses, physicians and the general public. - Works on multiple tasks at the same time. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: - Current Hawaii State license as a Pharmacist. - Doctor of Pharmacy degree preferred. - Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists preferred. B. EXPERIENCE: - Clinical hospital pharmacist experience preferred. Monitors pharmacy activities on a daily basis to ensure adherence to study protocols and dispensing procedures of controlled drugs in accordance with federal and state regulations.. Provides accurate, efficient medication dispensing. Ensure confidentiality of patient information and their records, and destruction of relevant documentation that contains patient information. Attends and contributes to interdisciplinary team meetings relating to clinical investigational drug trials. Participates in the development of software programs designed to facilitate management of clinical investigational drug trials. Collects and documents workload measurement statistics; provides other data, as required. Completes reports, including incident reports, adverse drug reaction reports, etc. Collaborates in the identification and development of revenue-generating opportunities, as deemed appropriate. Clinical Investigational Drug Trials Maintains responsibility for clinical investigational drug trials. Reviews protocols for University of Hawaiʻi Cancer Center clinical investigational drug trials, as required. Liaises with principal investigators and/or trial coordinators/research assistants to establish the pharmacy's role, determine fee schedules, and implement dispensing, compounding, and billing procedures. Communicates with and trains staff anticipated to participate in any aspect of the clinical investigational drug trial. Maintains a pharmacy electronic binder which contains a study summary, protocol, dispensing procedures, completed samples of required paperwork, dispensing checklist, fee schedule, billing procedures, and any other relevant materials for each investigational trial. Maintains and coordinates the distribution of an up-to-date list of clinical investigational drug trials, as scheduled. Maintains responsibility for the management of the inventory for clinical investigational drug trials; orders, replaces, and returns study materials, as required. Communicates any concerns regarding ethical issues or deviation from established University of Hawaiʻi Cancer Center policies & procedures to the pharmacist participating on the University of Hawaiʻi Cancer Center Ethics Committee. Assists in ensuring the participation of the pharmacy department in all clinical trials that involve drugs. Education and Communication Maintains professional development. Orientates staff on the pharmacy's role in clinical investigational drug trials. Promotes staff professional development; participates as an active member on committees; attends and/or presents at educational rounds, journal club, etc. Participates in the orientation and training of new employees and pharmacy residents, as required. Provides drug information to physicians, nurses, and other health care professionals, as related to clinical investigational trial drugs. Facilitates rapport between the University of Hawaiʻi Cancer Center, principal investigators, clinical trial coordinators, research assistants, and trial sponsors. Consults with other pharmacy personnel to obtain information/expertise in support of the investigational drug trials, as required. Effectively communicates with colleagues and allied health professionals. Liaises with external agencies/organizations, as required. Contributes information about clinical investigational drug trials to the pharmacy newsletter. Complies with all legal requirements and company policies. Performs all other duties as assigned. QUALIFICATION REQUIREMENTS Education/Training Minimum: PharmD from an accredited college or university. Completion of an accredited pharmacy postgraduate training program (PGY-1 and/or PGY-2 residency program). Board of Pharmaceutical Specialties certification in Investigational Drug Management, or 5 years of investigational drug experience Current and valid license with the State of Hawaii as a Pharmacist Current BLS/CPR Certification Within 30 days of hire, complete Collaborative Institutional Training Initiative (CITI) modules as assigned by the University of Hawai'i Cancer Center administration.
