Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.

The Clinical Staff Pharmacist performs in a hybrid capacity combining staffing and clinical/administrative responsibilities. The majority of staffing time is spent in a clinical/administrative practice site, such as, NICU, oncology, critical care, transitions of care, or the ED. The Clinical Staff Pharmacist is responsible for projects such as, policies, order sets, and performance improvements for their clinical area.
Formulary and P&T Protocol/Interchange Compliance: Achieves compliance with interchange, dosing, preparation and administration guidelines. Consistently identify, research and intervene on requests outside approved formulary, protocol or interchanges. If prescriber cannot be reached nursing staff is informed of issues and documentation is left for the next shift. Document interventions and cost avoidance per approved entity process. Successfully converts non-formulary prescribing to formulary-based alternatives. Takes initiative to communicate/educate staff regarding questions/issues via e-mail, direct communication, or pharmacist meetings, and provides follow-up and feedback to clinical specialists
Medication Safety and Performance Improvement: Communicate medication safety concerns to staff and leadership. Initiate and monitor new processes and protocols to ensure desired outcomes achieved Participate in collection of Performance Improvement Committee and Quality Assurance data. Participate in Performance Improvement projects, RCA's or FMEA's associated with relevant patient populations Respond to and report near-misses, errors, adverse medication events, including adverse drug reactions using the electronic reporting system. When an error or ADE occurs, assure the patient receives the appropriate antidote or other treatment to mitigate or avoid harm. Work directly with care givers in chosen practice site to review and develop strategies to avoid error-prone processes.
Patient Medication Management: Clearly communicates unresolved issues and hand-off to next shift. Interviews patients as needed to confirm therapy and allergy status, contacting family members or area pharmacies, if possible, to clarify medications. Communicates effectively with all customers to answer drug information requests, and documents per entity standards. Completes and documents (EMR and entity process) follow-up on all clinical consult patients. Consults with prescriber on medication management Documents cost avoidance for interventions using entity process. Interviews and/or counsels patients and family members on medication therapy as appropriate to assist with transition of care. Order review includes all elements per department standard, and assessment of system warnings and alerts. Participates in additional clinical services as assigned and needs of clinical service area Performs consults and follow-up for all approved pharmacy clinical services, including renal monitoring, kinetic dosing and other formalized P&T approved protocols. Proactively recommend medication therapy, monitor progress towards attainment of therapeutic goals, and initiate appropriate monitoring as needed Provides suggestions and supports implementation of improved processes for clinical services. Recommends appropriate dosing and monitoring parameters for specific drugs including initial and maintenance dosing as well as therapeutic endpoints based on available patient information as requested by prescriber Screen orders for safety and appropriateness of drug selection and dosage compared to age and condition of patients and protocols and profile, meeting department productivity standards.
Practice Management/Coordination: Current with new treatments and processes via the literature, professional associations and continuing education, bringing new ideas and practices to the Pharmacy. Establishes ongoing communication with staff through activities such as, regular rounding or communication with nursing staff and/or prescribers; reviewing P&T actions; in-services to nursing staff; or forwarding pertinent articles and drug information. Participates in nursing and prescriber meetings to discuss safety, order sets and implement new processes. Provides suggestions and supports implementation of improved processes and workflows. Responsible for review and revision of medication use policies and procedures Completion of new formulary requests for presentation to P&T. Serve as direct interface with nurses, prescribers, and other care givers, taking responsibility for problem resolution, as appropriate. Serve as pharmacy contact and clinical content expert for this patient population or service
Staff/Shift Management: Directs technician shift assignments to improve workflow during the shift. Monitors and guides technicians through their daily assignments. Assures that all assignments have been completed and that any pending work has been reported to the oncoming shift. Confirms timely and polite phone responses. Facilitates schedule changes as needed in the absence of the Operations Manager, Technician Supervisor or Director and communicates changes. Facilitates verification, reporting and resolution of automation/technical issues. Monitors all intravenous compounding activities to maintain compliance with USP 797 regulations. Readily verifies technician work to assure accurate and timely medication processing and delivery as designated in department workflows. Assure product preparation for bedside scanning.
Education/Meeting: Educates/trains new pharmacy staff and students. Activities can include serving as a designated preceptor for new staff during orientation; providing feedback/suggestions on evaluations; providing education to staff during Pharmacy Rounds; assisting with completion of staff competencies. Provides learning experiences for pharmacy residents.
Education
BS in Pharmacy or Pharm D required.
Certification & Licensures
Virginia OR West Virginia Pharmacy License required (or acquired within 90 days of hire). Must possess license for entity location.
Qualifications
Must be knowledgeable about medications, their pharmacology and dosage ranges for use in neonate, pediatric, adolescent, adult, and geriatric patients.

As a Pharmacist you will:
Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
