The Nuclear Pharmacist will compound and dispense radiopharmaceuticals. This job involves working in a clean room as well as occupational exposure to radiation. While reporting into Network Operations, the Nuclear Pharmacist role is instrumental to the Quality Assurance Program within SOFIE and will work closely with the QA division at the site-level.
Essential Duties and Responsibilities
Perform the duties associated with compounding, dispensing, and distribution of radiopharmaceuticals, including data entry with Pinestar for end-of-day reports and daily dose management reports.
Perform the duties associated with synthesis, quality control, and quality assurance of FDG and NaF, including reagent preparation.
Ensure compliance with USP , , proposed regulations, or other state pharmacy requirements as applicable, at the site level; maintain personal license in good standing through applicable state laws.
Ensure compliance to all applicable standard operation procedures (SOPs) and regulations, including 21 CFR Parts 211 and 212 requirements, by adhering to SOFIE's quality management system and maintain a state of cGMP control at the site.
Ensure aseptic operations follow internal procedures (gowning, cleaning/sanitation, sterility, environmental monitoring, etc.), customer requirements, and FDA regulations.
Ensure compliance with radiation and laboratory safety of the site, including the ability to address chemical or radioactive spills and reporting.
Complete cGMP documents as required and assist site and corporate Quality Assurance including, but not limited to:
o Investigations
o Corrective and Preventative Actions
o Deviations
o Out of Specifications
o No or Atypical Yields
o Manufacturing and QC Records
o Logbooks
Write and review internal procedures.
With site QA, review and release batch records as applicable.
Responsible for opening change controls and initiating, maintaining, and reviewing SOFIE network and site specific procedures relative to their job functions.
With site QA, monitor the metrology program at the site. Ensure that equipment is appropriately maintained, calibrated/recalibrated, or validated/revalidated in a timely manner.
With site QA, monitor customer/patient complaints to determine the possible root causes. Work with the customers to ensure their concerns are addressed, quality standards are met, and issues are resolved promptly and effectively.
Provide on-the-floor QA oversight of manufacturing, QC, and other cGMP activities. Identify non-compliance problems and propose solutions. Effectively interact with other functional personnel and resolve quality related issues on the spot whenever possible.
Provide quality customer service associated with radiopharmaceuticals to physicians, healthcare workers, customers, etc. as needed.
Train and supervise nuclear pharmacy technicians and maintain accurate training records.
Attend corporate and site level meetings as applicable.
Perform other duties as assigned.
Qualifications
B.S. or Pharm.D. from an accredited pharmacy school required; nuclear certification preferred.
-State license required (to be obtained post-hire if applicable).Background in nuclear pharmacy, Positron Emission Tomography, radiation safety and/or familiarity with cyclotron processes preferred.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations required.
Ability to write reports, business correspondence, and procedure manuals required.
Ability to effectively present information and respond to questions or complaints from groups of managers, clients, customers, the public required, or regulatory agencies required.
Ability to define problems, collect data, establish facts, and draw valid conclusions required.
Strong management and interpersonal skills required.
Proficient in all MS Office applications required. Proficiency in Pinestar or other pharmacy applications preferred.
Ability to be detail-oriented, responsible, dependable, patient, organized, professional and work in a team environment required.

Ability to communicate professionally with colleagues and customers, have excellent attention to detail, and be dependable and responsible in a fast-paced, highly technical environment required.

Major Duties
Oncology Formulary Management and Utilization:
Manage the oncology formulary by evaluating and onboarding new agents.
Determine the appropriate site of care for patients receiving oncology treatment, considering factors such as inpatient, outpatient, or regional campuses.
Oversee the review process for off-label, non-formulary, or other non-standard use of antineoplastic agents.
Promote appropriate, standardized and guideline-based antineoplastic utilization across all El Camino Health (ECH) sites where appropriate.
Review provider requests for exception orders and evaluate for literature support to guide the prior authorization process when applicable.
Collaborate with relevant stakeholders, including physicians and other healthcare professionals to ensure the safe and effective use of these agents.
Collaboration with Pharmacy Informatics Teams:
Work closely with pharmacy informatics teams to contribute to the development and revision of Electronic Health Records (EHR) resources.
Ensure that EHR resources support the safe and effective use of oncology medications and adhere to current guidelines and best practices.
Education:
Coordinate, facilitate, and provide education, training, competency assessment and performance improvement to pharmacists, pharmacy residents and other healthcare professionals for the appropriate use of antineoplastics as well as on best practices and the latest developments in oncology pharmacy.
Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.
Quality and Performance Improvement:
Contribute to the institution's antineoplastic stewardship program through compiling data and metrics on antineoplastic utilization in the outpatient and inpatient settings, reporting data and findings to relevant stakeholders.
Monitor antineoplastic utilization practices to identify areas for improvement and implement necessary changes.
Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.
Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.
Stay updated on the latest advancements in oncology pharmacy and ensure that the pharmacy services align with current standards of care.
Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all ECH sites and promote and enhance patient outcomes through safe, effective, cost-effective, and appropriate medication therapy use for patients receiving antineoplastic therapy.
Qualifications - All
Doctorate degree pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.
A one (1) year accredited PGY1 (Post Graduate Year) pharmacy residency.
A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with minimum of 2 years of oncology experience that includes pharmacy regulatory compliance, compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.
License/Certification/Registration Requirements/Credentialing
Valid California state pharmacist license with no disciplinary action(s) against the licensee that prohibits the licensee from practicing pharmacy.

JOB SUMMARY
Fills and dispenses prescriptions in accordance with current federal, state and long‑term care facilities. Supervises the pharmacy technicians to ensure medications are provided to the nursing staff and the residents in a timely manner. ESSENTIAL FUNCTIONS
Assures that drugs and biologicals are labeled and dispensed in compliance with federal and state laws as well as the facility's established policies and procedures.
Checks the prepared work of the filling pharmacy technicians and ensure that the prescription labels, medication dispensed, quantity, dosing and the resident's name are correct.
Performs Prospective Drug Regimen Review on all new prescriptions to monitor for renal drug dosing, maximum drug doses, drug/drug, drug/disease or drug/allergy interactions and communicates with prescriber if interaction is deemed significant.
Calls physician offices for refill requests or to clarify directions of new prescriptions
Answers pharmacy telephone calls and assists the residents or staff with refills and other medication-related questions
Provides oral consultation to residents with new prescriptions and/or refills as required by federal and state regulations if feasible.
Answers medication -related questions from the nursing staff and the physicians and work as a team to provide the best pharmaceutical services to our residents.
Assists with residents and staff at the pharmacy window with pharmacy needs.
Assumes all responsibilities for proper pharmacy operations in the absence of the Director of Pharmacy.
Enters new prescription orders and refills into the pharmacy computer system
Consults California Immunization Registry before dispensing any vaccines
Notifies Independent Living residents if there is a change in medication orders.

Transcribes new orders, change or discontinue orders on the med sheet of Assisted Living residents.
Assures that drug biologicals are stored in locked compartments and under proper temperature controls.

Counts and/or dispenses all Schedule II to V drugs accurately per each prescription order and maintain a perpetual inventory log. Report any narcotics discrepancy to the Director of Pharmacy immediately.
Enters drug orders in the wholesaler web sites if necessary
Arranges delivery of medications from after hour pharmacy for urgent medications that we don't have in stock if necessary.
Performs a walk-around in the pharmacy before closing the pharmacy and ensures all things are in order and no medications that need to be delivered that day are left behind
Marginal Functions
Collects prescription orders and delivers medications routinely and on an as needed basis to the Clinic, Assisted Living and Skilled Nursing.
Arranges for delivery of out‑of‑stock medications from back‑up pharmacy to be delivered to the correct nursing station.
Performs other duties as assigned by Director of Pharmacy.
Able to work flexible schedules and covers in the absence of Director of Pharmacy. Working 1-2 days per week on average
