The contract Health Care Provider (HCP) will work in conjunction with other health care providers, professionals, and non-contract personnel. The Contractor HCP shall provide the personal services of Clinical Pharmacist to perform services of the nature and scope described below. The quality of care provided by the Contractor HCP shall be of a quality meeting or exceeding currently recognized national standards as established by the Joint Commission (JC). Contractor HCP shall give the highest regard to patient dignity and observe the precepts of the American Hospital Association's Bill of Rights for Patients. The Contractor HCP shall abide by the MTF rules, regulations and by laws, including Medical Staff By laws, as well as applicable Army regulations governing such things as medical records. GENERAL: This contract is a personal services contract and is intended to create an employer-employee relationship between the Government and the individual contract health care providers only to the extent necessary for providing the healthcare services required under this contract. The performance of healthcare services by the individual contract health care providers under a personal services contract are subject to day-to-day supervision and control by healthcare facility personnel comparable to that exercised over military and civil service health care providers engaged in comparable healthcare services. Any personal injury claims alleging negligence by the individual contract health care providers within the scope of the health care provider's performance of the personal services contract shall be processed by Department of Defense (DoD) in the same manner as claims alleging negligence by DoD military or civil service health care providers. The contract does not create an employer-employee relationship between the Government and any corporation, partnership, business association or other party or legal entity with which the contract health care provider may be associated. The authority for this contract is 10 United States Code 1089 and 10 United States Code 1091. PROFESSIONAL QUALIFICATIONS / RESPONSIBILITIES: All Contractor HCPs shall meet the qualifications for that specialty and perform essentially the same functions, within the scope of acceptable for their technical professional discipline and standard, as those required by Army or government service technical professionals of similar experience and in similar duty assignments. Contractor HCP practices/productivity will be compared to that of another Contractor HCP assigned to the same medical center. a. The Clinical Pharmacist shall possess and maintain a current, active, valid, unrestricted license for the practice of pharmacy in at least one state or territory in the United States. b. The Clinical Pharmacist shall hold a post-baccalaureate or entry level Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy and at least one of the following: (1) Post-Graduate Year 1 (PGYl) Pharmacy Practice Residency from a program accredited by the American Society of Health Systems Pharmacist (ASHP). (2) Board certification in one or more specialties recognized by the Board of Pharmacy Specialties (BPS). (3) Minimum of 3 years of post-graduate experience as a clinical pharmacist with at least 50% of time spent in direct patient care related clinical activities such as medication therapy management, coordination of care, patient advocacy, wellness and health promotion, triage and referral, and patient education and self- management services. c. Shall have minimum of one year of successful clinical practice within the past three years. The time spent completing a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy may serve to meet this requirement. d. The Contract Clinical Pharmacist performing under this Contract shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers. e. Shall have and maintain current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS). Certification must be by American Heart Association and individual must demonstrate ability to initiate CPR in the event of respiratory or cardiopulmonary arrest. Certifications must be renewed biannually at no additional cost to the Government. f. Shall have transcript verifying level of education provided with resume. g. Shall demonstrate ability in the areas of interpersonal relations, problem solving, and conflict resolution. h. Shall demonstrate knowledge and skills necessary to provide care appropriate to any age-related needs of the patients. The individual demonstrates knowledge of the principles of growth and development appropriate to the patient population served: Infants, Children, Adolescent, Adults, and Geriatric (>65). i. Any Contractor HCP personnel found to be unacceptable in administering skills required by the Contract shall be immediately released from duty by the Contracting Officer's Representative (COR). The COR will immediately notify the Contracting Officer. SPECIFIC DUTIES/TASKS THE CONTRACTOR HCP SHALL PERFORM BUT NOT LIMITED TO: The Contractor HCP is responsible for providing medication therapy management services in collaboration with other providers to ensure that drug therapy is safe, effective, efficient, and cost-effective. This is accomplished by participating in the following activities: interprofessional patient care activities, medication reconciliation, prescribing and managing medication therapy, medication therapy monitoring, review of medication orders, patient education, providing drug information to other health care professionals, scholarly activity, coordinating drug distribution, pharmacy liaison to facility committees, overall quality assurance of pharmacy services, and performing other duties consistent with the scope of work, as required. The contractor must possess an extensive knowledge of medications, their indications, actions, and interactions to successfully perform these activities. U.S. Citizenship: Employee must be a U.S. citizen. For the purpose of base and network access, possession of a permanent resident card (“Green Card”) does not equate to U.S. citizenship. Place of Performance: Womack Army Medical Center, Fort Liberty, NC 28310 Hours of Performance: The performance hours of the MTF are as follows: The contractor shall provide services for work weeks averaging 40 work-hours per week. Possible shifts may be any consecutive 8 ½ - 10 ½ hours (including break) depending on the pharmacy section assigned. This schedule may include weekends, Federal and MEDCEN Training Holidays. Shift schedules may be adjusted to meet primary health care needs at the MEDCEN. On-call shifts are not required. Inpatient Pharmacy (24 hr operation): Inpatient shifts consist of an average of 4, 10-hour workdays plus a ½ -hour break, Sunday - Saturday on a rotating shift schedule. Shift hours are usually 0700-1930, 0900-1930, 1100-2100, or 1000-2000. Outpatient Services: Consist of an average of 5, 8- hour workdays plus a ½ -hour lunch break, Sunday-Saturday between the hours of 0800-2000. Outpatient Pharmacy Hours of Operation: Weekdays: 0800-2000 (usual shift is 0800-1700 or 1100-2000); Weekends/Holidays: 0800- 1630. “Mission Essential” and are required to report to work during Mass Casualties, Adverse weather, etc., as scheduled. Contract Pharmacist shall not have worked at another medical facility in the 8 hours prior to their shift at any MTF. Contractor shall not work more than 80 hours within a two week pay period. Overtime is not authorized. On call: For the purposes of this performance work statement, on-call shall be defined as when a contract HCP is assigned to carry a pager in order to be available after-hours, weekends, and holidays. The on-call rate is only for the contract HCP to carry the pager and be available to answer inquiries from patients and staff and/or report to the facility to see patients. Once the contract HCP arrives at the facility to see patients, then the hourly rate will apply, and the on-call rate will no longer apply. Once the HCP leaves the facility the on-call rate will apply. The contract HCP shall rotate on call with other HCPs at the MTF for the Department of Pharmacy.
