DUTIES AND RESPONSIBILITIES Preforms order entry, verifies order is entered and filled correctly, ensures prescription is accurately prepared for patient pick up. Counsels' patients or family members on the proper use, storage, potential side effects, and disposal of all medications dispensed. Orders and interprets laboratory tests per collaborative practice agreement. Addresses problem prescriptions in a timely manner prior to processing by addressing with the medical provider. Uses clinical judgement to extend refills or provide partial prescriptions until next scheduled appointment under the provider's name per facility collaborative practice agreement. Performs medication reconciliation during clinical interactions during patient care. Completes all assigned/required documentation or other assignments without intentional leave of work for others Reports metrics and measures in an accurate and timely manner Participates with collateral duties that benefit the operational success of the pharmacy. Demonstrates willingness to perform duties as deemed necessary by the supervisor. Prioritizes all duties appropriately to ensure efficiency. Actively participates in committees as assigned to promote pharmacy participation. Collaborates with other healthcare professionals and/or administration to develop protocols related to pharmacy Reviews and updates patient's medication profile, medication or dosage changes, allergy information, updates problem list, and monitors for compliance or potential abuse. Documents clinical activities and interventions with respect to outcomes. Reports adverse drug events, near misses, and medication errors. Recommends formulary alternatives as needed or requested. Provides leadership in identifying and developing cost avoidance strategies. Ensures that medication related issues are resolved as soon as possible to provide optimal care. Conducts prescription drug monitoring program review for every controlled substance prior to processing and document if there are findings or not in chart. Selects and administers appropriate vaccines to healthy adult (18 and older) patients. Responsible for maintaining professional competency by keeping abreast of current medical and pharmaceutical literature, new developments in pharmacy practice and new drugs and treatment plans and applies knowledge in daily duties. Provides education to patients, health care providers, other pharmacy staff/students, and support staff regarding medication use and or preparation. Conducts just in time and on the job training as needed. Performs other duties as assigned. QUALIFICATIONS: Graduation from an accredited college or university with a degree in pharmacy. Licensure as a registered pharmacist from a state, the District of Columbia, or a territory of the United States CPR Certification or ability to obtain within six months of hire. Certification in Pharmacy-Based Immunization Delivery or ability to obtain within six months of hire Communication: Excellent verbal and written communication skills Computer Skills: Word processing, spreadsheets, pharmacy software, and databases APPLICATION REQUIREMENTS: Submit a completed application with supporting documents to Warm Valley Health Care, Human Resource Department, P.O.
