Summary:
Children's Health Specialty Centers for outpatient therapy offer a comprehensive range of pediatric therapy services aimed at supporting children's physical, developmental, and emotional needs. Our services include care coordination, developmental screenings, occupational therapy (OT), physical therapy (PT), speech/language therapy, parent education and support, and specialized treatment programs. We provide tailored services such as transitional skills evaluations for children ages 11-17, focusing on developing independence in self-care, work skills, and functional abilities.
In addition, we offer specialized assessments, including augmentative communication evaluations and feeding therapy for children with neurological and behavioral disorders. Our therapy programs also address handwriting disorders, sensory integration, and neuromuscular electrical stimulation, among others. Group classes are available to enhance patients' abilities in areas like articulation, feeding, motor skills and social needs.
Responsibilities:
Provide direct Physical Therapy evaluations and re-evaluations, including assessment of prior and current level of function, setting individualized goals and making discharge recommendations in accordance with standards of practice; provide direct PT treatments according to standards of practice, including patient and family education, progression of treatment plan and implementation of appropriate therapeutic interventions; report on patient progress through appropriate written documentation including evaluation forms, progress notes, education forms; and collaborate with physicians, patients, family and other members of the health care team relative to patient treatment and progress as well as other affiliated agencies with regard to effective discharge planning
Coordinate and prioritize own schedule in relation to demands of patient treatment and other departmental responsibilities; supervise rehabilitation aides; mentor and instruct PT interns and other students as indicated
Demonstrate proper use and maintenance of department equipment and supplies; attend and participate in departmental meetings, in services, rounds and peer reviews; participate in program development as assigned by Team Leader; and participate in continuing education programs or other opportunities for learning for continued professional competency
Ability to meet physical and non-physical demands as outlined in the job description
Assume other duties and responsibilities that are appropriate to the position and area

POSITION QUALIFICATIONS
Bachelors of Social Work with 1 year of applicable experience required; Master's degree preferred
Texas State license required
Reliable transportation and valid driver's license.
Hospice or healthcare experience preferred.
Proficient skill using a computer is required

Summary:
This position specializes in the psychiatric evaluation and care of children and families in crisis. Primary responsibilities include psychiatric assessment and intervention of patients in the emergency room. This position may provide support for other areas within the System that have patients with psychiatric needs. These services may include any of the following: diagnostic evaluations; consultations on the behavioral management of medically and psychiatrically ill children; case management and disposition planning as appropriate. This position also provides education and practical coping strategies for patients and families. Communicates with patients, families and the multi-disciplinary team, to ensure continuity of care and collaborative treatment planning 24/7.
Responsibilities:
Provide evaluation and treatment services to patients and families with complex psychosocial needs; provide crisis intervention and emergency services as needed
Perform assessment and intervention for individuals presenting to the emergency room with a chief complaint of mental health concern or identified secondary complaint of mental health concern with special emphasis on risk of suicidality and safety
Provide patient care and treatment information throughout the shift and across shifts with ongoing communication to both medical and psychiatry care team partners
Case management and disposition arrangements
Contribute to program development
Maintain fiscal accountability and apply problem solving skills that demonstrate responsibility for generating allocated billable and/or patient contact hours
Contribute to the teaching mission of the hospital by working with trainees as co-therapists, and participating in didactics on topics of own expertise
How You'll Be Successful:
WORK EXPERIENCE
At least 1 year job related experience required
At least 3 years job related experience preferred
Psych Assessment experience at an inpatient psychiatric facility or ER setting preferred
Experience in suicidal ideation assessments preferred
EDUCATION
* Graduate or professional work or advanced degree; or equivalent experience required
LICENSES AND CERTIFICATIONS
* State of Texas independent license at a Master's level (LPC, LMFT, LCSW) required
* Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements from the American Heart Association within 60 Days, required

What You Will Do\:
Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner:
Completes Transition Evaluations and collects Social Determinants of Health (SDOH) data on patients within 48 hours of identification and begins discharge planning. Assesses and interviews patient and caregivers as part of this evaluation and as needed.
Reviews the Risk of Unplanned Readmission (RUR ) scores daily for all assigned patients.
Assists in the identification of a primary care physician (PCP) for patients without a PCP and attempts to schedule follow up appointments with either a PCP, specialist, clinic, visiting physician or other transitional care visit prior to discharge.
Identifies transition needs and discusses funding of post-transition care with patients and caregivers.
Participates in multidisciplinary rounds (MDR? s) to help identify current length of stay (LOS), expected discharge date, anticipated discharge disposition, barriers to discharge, avoidable days, and potential denials.
Coordinates/facilitates patient care progression throughout the continuum with the objective of enhancing quality clinical outcomes and safe discharge planning to the appropriate levels of care for high-risk patient populations.
Coordinates with patients and families to manage chronic conditions and ensures appropriate post-discharge clinical follow up.
Proactively identifies patients who no longer meet continued stay criteria and communicates with the physician team.
Assigns patients to and supports appropriate transition programs (e.g. ACO members) when applicable
Updates and executes the discharge plan as needed.
Communicates final transition plan 24-48 hours prior to transition.
Facilitates care conferences for complex transitions, placement, and palliative care needs.
Serves as a point of contact for all identified stakeholders.
Proactively identifies and documents barriers to discharge while working to resolve them, including obstacles impeding diagnostic or treatment progress.
Assists patients who have complex psychosocial needs; offers solution-focused interventions to patients/families when needed.
Assists with eligibility determination for funding sources and other community resources including housing, food and mental health services.
Collaborates with Palliative and Pastoral Care to provide end of life and grief support and facilitates referrals to the appropriate agencies.
Provides intervention in cases involving child or elder abuse/neglect.

Reviews care options and, as appropriate, utilizes existing protocols/processes to facilitate continuity of care within the Texas Health network and to ensure prompt and convenient scheduling of follow up appointments.
Schedule/coordinate patient clinical needs to the appropriate post-acute care facility based on facilities? clinical capabilities/offerings, historical quality outcomes results, preferred network, and patient informed choice
Identifies community resources and service needs and facilitates appropriate referrals as needed, while also providing education to patients, caregivers, and the multidisciplinary team regarding the available post-acute care services and needs.
Assist with referrals for community resources including housing, food, transportation, and other social and environmental issues affecting health.
Serves as a content expert regarding payor information. Educates the multidisciplinary team, patients and caregivers regarding payor requirements and barriers. Communicates with payors as needed to coordinate care.
