Job Title: Behavioral Health Clinician (LPC, LMFT, or LCSW)

JOB SUMMARY: To perform a combination of behavioral health clinical tasks including and not limited to intake assessment on referrals for mental health services and/or provide counsel with emphasis on prevention and recovery as assigned on weekly rotation schedule by Program Manager. Work with individuals and groups to promote optimum mental health. This position will help individuals deal with complex and comorbid diagnosis which may include: serious persistent mental health illness, addictions and substance abuse; family, parenting, marital problems and challenges with social determinants of health; suicide; stress management; problems with self-esteem; and issues associated with aging, physical, mental and emotional health.
RESPONSIBILITIES:
Perform face-to-face intake for consumers who have been referred to MHC and assess for care management, medication management or therapy eligibility.
Complete all intake evaluation paperwork: initial intake, and grief when needed and any other necessary paperwork.
Determine an individual's appropriateness for care management, medication management or therapy and assign to all that are appropriate.
Disposition consumer to appropriate Agency for other mental health services when ineligible for care management.
Identify individuals that are in need of MCRT services rather than a routine intake. If appropriate, facilitate crisis action.
Assist in resolving any grievance received from a consumer regarding referral.
Fill in for Admissions Coordinator as indicated by supervisor.
Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review. Contact consumer with decision.
Utilize evidence based therapeutic framework, counsel consumers and patients, individually and group sessions, to assist in overcoming dependencies, adjusting to life and making changes.
Guide consumers in the development of skills and strategies for dealing with their problems using evidence-based modalities.
Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
Develop and implement treatment plans based on clinical experience and knowledge.
Modify treatment activities and approaches as needed in order to comply with changes in consumer's status.
Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
Counsel family members to assist them in understanding, dealing with and supporting consumers.
Evaluate the effectiveness of counseling programs and consumers' progress in resolving identified problems and moving towards defined objectives.
Refer patients, consumers or family members to community resources or to specialists as necessary.
Meet with families, clinic providers, care managers and other interested parties in order to exchange necessary information during the treatment process.

JOB SUMMARY: A full-time licensed behavioral health provider to include LMFT, LCSW, and LPC/MHSP who provides behavioral health counseling to HOPE patients. This position will assess clients and provide them treatment utilizing a variety of evidence-based counseling or therapy techniques to help guide and support optimal health functioning in each client's life. The program serves predominantly clients who exhibit symptoms of behavioral health disorders such as Depression, Anxiety, PTSD, Bipolar Disorder, and general mental health concerns, as well as persons who exhibit symptoms of substance use disorders.
PRIMARY DUTIES AND RESPONSIBILITIES:
Provide screenings and initial behavioral health assessments
Provide short-term, structured, evidence-based psychotherapy (individual, family, and group) as part of a comprehensive integrated treatment plan in accordance with Federally Qualified Health Center (FQHC) and State requirements
Input behavioral health goals, objectives, and interventions into integrated treatment plans
Attend treatment team meetings and coordinate care with the Director of Behavioral Health and HOPE's primary care providers to improve health outcomes
Participate in clinical supervision with the Director of Behavioral Health
Facilitate referrals for other services (e.g. substance abuse treatment centers, community mental health centers, etc.) as needed
Serve on such committees as directed by senior management

OFF-SITE WORK:
Off-site work is required and may travel between the Westmoreland, Gallatin, and Macon County-Westside office locations.

JOB SUMMARY: A full-time licensed behavioral health provider to include LMFT, LCSW, and LPC/MHSP who provides behavioral health counseling to HOPE patients. This position will assess clients and provide them treatment utilizing a variety of evidence-based counseling or therapy techniques to help guide and support optimal health functioning in each client's life. The program serves predominantly clients who exhibit symptoms of behavioral health disorders such as Depression, Anxiety, PTSD, Bipolar Disorder, and general mental health concerns, as well as persons who exhibit symptoms of substance use disorders.
PRIMARY DUTIES AND RESPONSIBILITIES:
Provide screenings and initial behavioral health assessments
Provide short-term, structured, evidence-based psychotherapy (individual, family, and group) as part of a comprehensive integrated treatment plan in accordance with Federally Qualified Health Center (FQHC) and State requirements
Input behavioral health goals, objectives, and interventions into integrated treatment plans
Attend treatment team meetings and coordinate care with the Director of Behavioral Health and HOPE's primary care providers to improve health outcomes
Participate in clinical supervision with the Director of Behavioral Health
Facilitate referrals for other services (e.g. substance abuse treatment centers, community mental health centers, etc.) as needed
Serve on such committees as directed by senior management
Provide crisis intervention services as needed
Maintain accurate and timely records of activities and services
Assure the security and confidentiality of all protected health information as required by state and federal law
OFF-SITE WORK:
Off-site work is required and may travel between the Westmoreland, Gallatin, and Macon County-Westside office locations.
This position incorporates the use of telehealth for patient appointments depending on the schedule. All rules and regulations are expected to be followed regarding HIPPA compliance.
Employees who participate in off-site work are required to have a confidential designated workspace when completing job tasks.
SKILLS/QUALIFICATIONS:
Master's degree in Social Work, PhD in Clinical Psychology or a Master's degree in Marriage and Family Counseling or Clinical Mental Health Counseling
Current licensure as Licensed ClinicalSocialWorker or Licensed Psychologist in the State of Tennessee (other licenses will be considered), LMFT. LPC-MHSP, or LMSW, supervision offered.
Excellent knowledge of behavioral health care, psycho-pharmacology, and evidence-based treatments for health conditions
Familiarity and comfort working with issues related to homelessness, substance abuse, domestic violence, and history of trauma

How you'll contribute
A clinical program therapist who excels in this role:
Coordinates, develops, and delivers curriculum to meet the psychological needs of patients.
Participate within a lead role in the multidisciplinary treatment team planning process, encouraging patient and family involvement.
Perform clinical services aimed at stabilizing the psychiatric crisis and the development of healthy mental health management through a biopsychosocial model of treatment.
Responsible for coordination of discharge plan through discharge planning meetings on assigned units and follows up with patients, family, physician, and hospital staff from admission through discharge.
Initiates patient and family education and discharge planning at admission.
Arranges for discharge and post-hospital care of patients through institutions and agencies within the community.
Coordinates the referral of patients and families to appropriate community, social service, home health service, extended care, and rehabilitation facilities, and organizations.
Creates appropriate discharge plans and documents information in the medical record.
Assists in identifying opportunities for improvement, participates in data collection and analysis, contributes to problem solving and identification of actions to be taken and effectiveness of those actions.
Utilizes evidence-based tools to analyze and identify trends/patterns of performance through variance reporting to improve quality and satisfaction and decrease variation and cost.
Prepares statistical reports on department services and patient needs; participates in the billing procedures for services rendered as required.
Provides education and support to Clinical Staff by attending staff meetings, small groups, and one-on-one education related to resource utilization, discharge planning, and psychosocial aspects of healthcare delivery.
Facilitates group psychotherapy and facilitates psychoeducational focus groups serving as a role model for patients to observe and learn socially effective values, attitudes, and behaviors.
Documents patient care and information in an accurate, legible, manner utilizing correct grammar and spelling; documentation is reflective of patient behavior, thoughts, feelings and responses to milieu and the treatment plan directed interventions.
Interactions with others are nonjudgmental, assertive, and respectful demonstrating a professional, caring attitude and behavior towards others; behaviorally demonstrates understanding of program philosophy, rules, and regulations.
Offer brief individual, group, and family therapy, psychoeducation, crisis intervention, as well as providing information and referrals as needed in collaboration with the case management department.
Reviews charts in preparation of patient treatment planning and provides input on patient progress.
Evaluates and updates treatment interventions in collaboration with charge nurse and/or Program Director.
Demonstrates knowledge of the principles of growth and development over the life span of the patient to include child, adolescent, adult, and geriatric stages.
Serve on weekend assessments, group therapy rotation, and clinical-on-call rotation when needed.

What we're looking for
Applicants should have:
Master's Degree in Social Work, Marriage and Family Therapy or equivalent Professional Counseling pending State requirements.
Licenses/Certification: · LMSW, LCSW LPC-I, LPC, LMFT Independent Practice Scope Licensure in State of Practice preferred.
Experience: · Minimum 1 year in discipline preferred.

Requirements
Must have a Master's degree in Social Work
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).

Must maintain current licensure to practice as an LCSW
