Day to Day Responsibilities of this Position and Description of Project:

Assist member with filing and resolving complaints and appeals.

Position Specific Information
This position is public facing and may be located between 3205 Freedom Drive, Charlotte, NC 28208 and between partner locations in which people seeking shelter may attend. Standard hours of Sunday - Thursday from 8:00 am to 5:00 pm. Subject to change, based on need.
Mecklenburg County is the Lead Agency for Coordinated Entry (CE) for the Charlotte-Mecklenburg Continuum of Care. Coordinated Entry is the intake for the local homeless response system. It is part of the process to access homeless resources and shelter in Charlotte-Mecklenburg.
This SocialWorker I position conducts Coordinated Entry (both in person and telephonically) with individuals and families experiencing homelessness, or those at imminent risk of homelessness, to efficiently connect them to the best available (scarce) homeless shelter, temporary housing, or homeless prevention options in the community. It may require the provision of short-term Systems Navigation support to clients until which time they are connected to a homeless services case manager and/or their situation resolves.
This position also engages clients within a strengths-based, solution focused framework to identify their service needs and goals, and to facilitate referrals to community resources so that clients have a plan of action toward enhanced self-determination.
CE clientele is diverse and brings with them a wide range of experiences and concerns; and may be in different stages of mental health, substance use, and trauma recovery.
In addition to clients, the SocialWorker I requires the ability to effectively engage with community partners to provide on-site CE to clients wherever they may be (in places not meant for human habitation, hospitals, treatment centers, day centers, shelters, etc.)
Position Summary
Provides community-based services to clients, including evaluation, case management, counseling, and referrals. This position, under direct supervision, is responsible for determining the individual's specific problems and assisting in developing an action plan and/or mutual responsibility agreement.
Essential Functions

Minimum Qualifications
Experience:

* Assesses reports of abuse, neglect, or dependency of children using either a forensic investigative assessment approach or a family assessment approach, depending on the severity of the maltreatment reported, and in accordance with state law and divisional policy governing such. Preliminary reviews existing DSS documentation (if such exists) and staff response process with the Supervisor. Conducts a comprehensive assessment including interviews of all victim children, all caretakers, all person living in the home, all appropriate collaterals (as indicated by both the reporter and the family being investigated), and all other persons, entities, agencies, professionals, relatives, etc. who might have knowledge about family relationships, behaviors, and incident (or incidences) that prompted the report to DSS. Follows mandated timeframes and procedures for appropriate initiation and continuous process of assessment. Observes the home structure and environment, ensuring that persons, provisions, and location are able to meet the basic needs of each child involved in the report. Completes safety assessments (initial and final) and all structured documentation tools with the family to promote a transparent process. When necessary, explore alternative safety resource placement options with the family to avoid removal of physical or legal custody. Engages the family in planning for safety and well-being, encouraging immediate action and front-loading services. Incites the professional opinions of necessary contributors (law enforcement, medical personnel, mental health professionals, etc.) in order to determine what safety measures are necessary adequately.
* Invokes court action when children cannot be safe in their homes, seeking direction through legal consultation, and completion/filing of all necessary court paperwork. Accompanies law enforcement in taking physical custody of child(ren). Collaborates with internal licensing/placement experts to identify and locate placement that best meets the child's needs and physically delivers the child to the caretaker.
* Participates in forensic joint interviews at the local Child Advocacy Center on appropriate cases (with law enforcement and any other appropriate professionals).
* Participates weekly in unit/team staffing, presenting completed case assessments for unit/team decision-making. Solicits guidance, direction, and support from the supervisor individually at any point in case and/or requests assistance in completing tasks, if needed.
* Documents all activities, contacts, dates, and times on a structured documentation tool in NC Fast. Requests necessary supporting documentation from external sources (i.e., medical/hospital records, police reports, 911 response sheet) and places such in the case file.
* Participates in local, professionally-enhancing, and state-mandated trainings. Participates in monthly MDT meetings to discuss specific cases in a multi-disciplinary setting. Participates in quarterly internal meetings with all fellow assessors to discuss policy changes, service gaps, equipment needs, process instruction, etc.
* Participates in a secondary backup system for after-hours on call, covering a one-day slot during periods of illness, vacancy, or emergency by regular after-hours staff. Must be available to work mandatory emergency shelters and perform other duties as assigned.

Serves as a resource to the college's faculty and staff by informing the campus community of various resources for students. The SocialWorker works in collaboration with campus and statewide stakeholders and they are essential in identifying resources, following up on needs and resources and assisting students to persist to graduation.
* Student Support and Other Resources: Assist in connecting students with campus and community resources that assist with resolving social, economic, emotional and behavior challenges. Provide ongoing support to students to assist students with their ability to cope with problems or other issues related to food insecurity, housing issues, financial well-being and/or family crises.
* Student Community Resource Center Support: Communicate, coordinate, refer and track services for students who utilize the Student Community Resource Center (SCRC). The Social Impact Intern will be responsible for assessing the impacts of Equitable non-profit partnerships, while also participating in the activation and communication of established programs. The socialworker provides service intake several times monthly which includes submitting CAP referrals and the ability to educate the community on adult service programs and community resources. JOB RESPONSIBILITIES: Provide Child Protective Services Case Management Services to a caseload of children and families where services have been found to be needed or reasons found to substantiate neglect or abuse; develop Family Service Case Plans; participate in Child and Family Meetings; present cases in court when needed; other duties and types of cases as may be required.
SPECIAL ATTRIBUTES AND SKILLS: Thorough knowledge of social work principles, techniques, and practices and their applications to complex casework, treatment, and investigation of abuse or neglect of children; thorough knowledge of policies and procedures as evidenced by the ability to cite the authority of federal and state law; thorough knowledge of individual and group behavior, family dynamics, and medical, behavioral, and/or psychosocial problems and their treatment theory. Considerable knowledge of governmental and private organizations and resources in the community. Ability to interact with and motivate a resistant involuntary client population and the public who may not agree with the laws, rules, or policies of the process or the programs; ability to prepare documentation such as written investigative reports for the court, case records, and treatment plans; ability to testify as an expert witness; ability to employ advanced case management interview techniques to establish a supportive relationship and involve families in the initial assessment for the need of services; ability to quickly assess the risks and safety of the client environment during daylight hours, after dark, and in high crime areas; ability to employ expert negotiation skills in the most complex cases; ability to analyze and assess child development safety issues in relation to risk factors; ability to analyze tense family situations and make decisions about removing children when the decision has to be made with limited direct information and limited access to consultation; ability to communicate effectively and establish supportive client relationships. Ability to perform manual work exerting up to 50 pounds of force occasionally and/or up to l0 pounds of force constantly to move objects.

$28.55 - $42.85 Provides crisis intervention and information/referral sources to patients and the patient's social support system. Works collaboratively with the Clinical Care Management team to effect positive patient and family outcomes and to ensure efficient utilization of resources.
Essential Functions
Provides casework services to patients and patient's social support system for a designated patient population. Assists patients and patient's social support in dealing with issues related to hospitalization and disease processes.
Interviews patients and patient's social support system when appropriate to identify and assess needs, evaluate resources, and work with patient, patient's support system, and the multidisciplinary team to develop a post hospital plan of care.
Provides psychosocial assessment with the patient and develops a post acute plan of care/discharge in coordination with the Clinical Care Management team and the interdisciplinary team.
Coordinates and facilitates post hospital planning for patients and patient's support system in conjunction with the Clinical Care Management team.
Works collaboratively with the patients and patient's social support system to establish and implement a plan of care with multidisciplinary team that meets the patient's identified psychosocial needs.
Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness and its effects on their social support system. Assists and facilitates the multidisciplinary team in understanding and integrating these aspects into the plan of care.
Intervenes in crisis situations, appropriately and effectively applying crisis intervention theory and skills
Works with the Clinical Care Management team to assess and evaluate the patient/patient's social support system for the appropriate level of care or environment setting to meet care needs across the continuum.
Maintains current knowledge and researches availability of community agencies and community resources for social, emotional or financial assistance.
In collaboration with the Clinical Care Management team, provides information, education to patients and their social support on community resources and options for post hospital care appropriate to the age of the patients served. Make referrals to community agencies as needed.
Reports suspected cases of child & adult abuse/neglect/exploitation. Serves as liaison between hospital, patients/patient's social support system and Department of Social Services during evaluation/investigation.
Works with Clinical Care Management leadership to identify problems, recommended solutions and works toward resolution and efficient discharge planning.
Physical Requirements
Works in an office type setting, extensive walking throughout the facility. Prolonged periods of sitting reviewing medical records, documentation. Repetitive wrist motion and occasional lifting of 10-20 pounds. Requires frequent verbal and written communication in English. Intact sight and hearing with or without assistive devices is required. Must be able to handle fast-paced environment, moving independently from one location to another.
Education, Experience and Certifications
Bachelor's Degree in Social Work. North Carolina Certification preferred. At least 1 year professional experience in hospital or health related setting preferred. Experience with Word and Outlook preferred. Adherence to National Association of SocialWorkers Code of Ethics.
