Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
Overview of the Role
We want to hire bachelor's prepared SocialWorkers (or another related field such as Gerontology) who will work with a Care Management Team (CMT) consisting of Master's or Bachelor's prepared SocialWorker and Registered Nurse team to provide comprehensive case management services to target population.

You like working 8am-5pm on weekdays with no weekend work.
Responsibilities:
Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participant's needs
Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the community
Responds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordination
Provides education to participants so they can better understand the benefits of services and community supports
Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of support
Document all consumer interaction and maintain timely reporting and documentation requirements
Observes participants behavior; reports significant changes to supervisor or team; documents appropriately
Daily case conference with CMT about shared participants
Performs other related duties as assigned
Essential Requirements:
Master's degree or Bachelor's degree in Social Work or another related field
Skilled in operating a personal computer utilizing a variety of standard software
Valid California Driver's License and eligible to be insured under our liability policy with a clean DMV report
Your own vehicle and proof of current auto insurance
CPR certificate required within 30 days of hire
Preferred Skills, Knowledge and Certifications
Knowledge of basic problems and needs of people diagnosed with a physical disability or emotional disturbance
Knowledge of community and supportive services
Skilled in assessing and prioritizing multiple tasks, projects, and demands
Skilled in working within deadlines to complete projects and assignments
Skilled in establishing and maintaining effective working relations with co-workers and other community providers
Experience with electronic medical records
Location and Service Region:
This is not a remote role and you are required to work from our Chico office.
You will need your own car and be able to travel to client's homes in Butte County and surrounding counties such as Glenn and Tehama. Work is performed in a community and office environment.
PPE is provided to all employees.
Transporting clients with personal vehicle is prohibited
Schedule:
This position is full-time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.
Physical Requirements:

Compensation:
Pay range is $22 to $30 per hour. Exact compensation may vary based on skills and experience.

POSITION SUMMARY:

Knowledge of current medical terminology to communicate with physicians, staff, and patients

Skilled in analytical decision-making and the ability quickly assess and resolve complex logistical, technical, and patient care problems and issues.

Minimum Qualifications
High School Diploma (required).
Minimum one year of clinic technician experience (required).
COT (Certified Ophthalmic Technician) / COA (Certified Ophthalmic Assistant) Certifications (preferred).
Physical Requirements/Typical Working Conditions:
Environment: Primarily a temperature-controlled facility.
Work may require prolonged standing, some bending, stooping, and stretching.
Requires hand-eye coordination and manual dexterity sufficient to operate surgical equipment, a keyboard, mouse, and standard office equipment.
Requires normal range of hearing and eyesight to communicate in person, by phone, or over video.
Requires occasional lifting papers or boxes up to 30 pounds.
Essential Physical Tasks:
Sight - sufficient to adjust small instruments, read physical documents, and images on a computer monitor.
Smell - sufficient to discern smoke/something burning, and other odors.

Expected Hours of Work: This is a full-time position. Core days and hours are Monday through Friday, 8:00a.m. to 5:00p.m. The Clinic Supervisor is expected to effectively balance administration hours and in clinic hours to ensure the essential functions and responsibilities of this position are being performed successfully.

POSITION SUMMARY:
Under the general supervision of the Clinic Medical Director and Director of Clinic Operations, the incumbent is assigned to the Colusa Indian Health Clinic. The incumbent provides counseling services to individuals, families, children, youth and adults. In addition, the therapist is responsible for providing assessments, diagnoses, and individual and group counseling services.
The incumbent is expected to work in accordance with the
mission, purpose, and values of the Colusa Indian Community
Council.

ESSENTIAL DUTIES and RESPONSIBILITIES:
Participates in the intake process and the evaluation of problems presented by clients.
Develops appropriate treatment plans.
Conduct pre and post assessments related to emotional health needs;
Provide training to clinic staff in the area of mental health;
Address Behavior Health referrals from Clinic Providers;
Meet with individuals and or family members to assist them with functioning more effectively;
Meet with Health Clinic patients to observe behavior, assess needs and creates appropriate treatment recommendations;
Provide therapeutic services to patients and their families;
Interview individual or family members to obtain information concerning behavioral health history, treatment and other pertinent information;
Educate patients and their families about mental illness, treatment plans and resources;
Facilitate regular multidisciplinary review meetings for the purpose of planning care and assessing the effectiveness of therapeutic interventions, group therapy process and program components;
Implement and evaluate individual or group counseling treatment needs;
Discuss progress toward goals such as medication regiment, family relationships, educational programs and other related problems;
Complete and maintain individual and or group counseling notes and documentation;
Provides crisis services as needed.
May perform other duties commensurate with the functions and level of the position.
MINIMUM QUALIFICATIONS:
Education:Masters degree from an accredited college or university in clinicalsocial work or closely related study. Postgraduate experience in acute outpatient mental health services, with specific experience in individual and group process counseling.
Experience: One (1) year experience in a clinical setting.
Licensing: Current LCSW or LMFT licenses from the State of California; Current Basic Life Support (BLS) Certification.
