Careline Hospice Medical Social Workers are trained professional Social Workers who assist patients and families living with terminal illnesses. The Medical Social Worker coordinates care to enhance the patient's quality of life by handling the day-to-day details of the client's medical care, establishing, implementing, and evaluating goals, and counseling the patient and their family/caregiver as they journey through the end stage of life.
In this role you will be responsible for:
Completing the patient's initial evaluation within 5 days of hospice admission and re-evaluating the patient's social work needs during each subsequent visit
Assisting patients and caregivers in planning for funeral arrangements, financial, legal, and health care decisions
Educating and assisting patients and caregivers in the preparation of advanced directives
Providing information to patients and caregivers regarding community agencies and referral services
Carrying out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect, or abuse, and planning intervention based on evaluation findings
Assessing the psychosocial status of patients and caregivers related to the patient's terminal illness
Counseling patients and caregivers as needed concerning stress, and other identified coping difficulties
Assessing for and educating staff on any special needs related to the culture of patients and their caregivers. This includes communication, the role of family, space, and any special traditions
Providing bereavement support to patients, caregivers, and hospice personnel
Required Qualifications
Master's degree in Social Work from an accredited college or university
Current state licensing in social work
Current and valid driver's license
Reliable transportation and valid automobile insurance coverage
Minimum of one year experience in healthcare
Medical Social Work skills are as defined by accepted MSW standards
Understands the hospice philosophy, and issues of death/dying
Experience and or education in grief or loss counseling
Demonstrates ability to assess and respond to the needs of patient's families and caregivers in varied settings and to function effectively in the home or nursing facility environment

The Outpatient Clinician is accountable to the Outpatient Clinical Supervisor to provide therapy and casework (where applicable) to clients they are assigned within a wide range of presenting concerns and diagnostic criteria within the office setting.
Compensation Range (based on licensure): $62,000-$75,000

RESPONSIBILITIES:
Outpatient:
Conduct assigned intake assessments for counseling, completing all required paperwork.
Provide individual, family and/or multi-systems therapy for assigned clients.
Maintain appropriate and timely progress notes and files on each client.
Participate in individual supervision with the Outpatient Clinical Supervisor.
Participate in bimonthly group team meetings presenting on cases and clinical topics as required. Participation includes the sharing of ideas, feedback, and department planning. Seek and solicit feedback as needed.
Continually assess client's needs and make appropriate referrals if additional services or a more intensive level of care is needed.
Develop Treatment Plans, Quarterly Reviews, Discharge Summaries, Clinical Progress Notes, and other documents as required by contract.
Coordinate the transfer/discharge process for the client and family, informing Outreach Clinical Supervisor, and necessary support staff of projected transfer/discharge goals.
Monitor the quality of overall treatment services to clients on their caseload; and evaluate the adequacy of these services to meet treatment objectives. Communicate effectively with Wedgwood's Psychiatric services regarding clinical as well as case management/financial issues as needs arise.

Responsible for timely Quality Assurance compliance as related to employee's roles and functions.

Community Based:

QUALIFICATIONS:
Master's degree in Social Work, Psychology or a related field, from an accredited institution.
Registration and continuous training completion toward a Certified Advanced Alcohol and Drug Counselor (CAADC) Development Plan from Michigan Certified Board for Addiction Professionals (MCBAP)
Experience providing individual, family and group counseling, preferred.
Required to provide proof of full license in one of the following areas: Social Work, Psychology, Counseling or Marriage and Family Therapy.

The Corewell Health West Ambulatory Care Management Department is made up of Social Workers, RN Care Managers, Community Health Workers and Medical Assistants. We have a large team of Social Workers and RN Care Managers who are embedded in Primary Care offices across the system. Care Management work is focused on identifying and connecting with patients who have behavioral health concerns or chronic medical conditions on an outpatient basis and assisting these patients with developing healthy behaviors and coping skills that will improve overall health outcomes. Our Ambulatory Care Managers play an active role on the patient's care team and partner with primary care providers in developing an appropriate treatment approach for patients experiencing chronic disease and/or behavioral health concerns to promote self-management and symptom improvement. The Ambulatory Care Management team also provides comprehensive transitions of care support to patients who have been admitted to the hospital in an effort to keep patients well in the community, prevent readmissions and reduce overall cost of care. Resources and support for barriers related to Social Determinants of Health (SDoH) is also provided to primary care patients by our remote team of Community Health Workers. ESSENTIAL FUNCTIONS:
Serve as a case manager or co-writer with the teacher of all student assessments and learning plans that focus on educational needs, as well as other needs including IEP goals/accommodations as applicable that may affect a student's functioning in school. This may be for example housing assistance, mental health/substance abuse, job skill/career exploration, etc. Ensure these documents are completed in a timely manner.
Provide individual SSW services/therapy to each student/family on his/her caseload, as needed, or coordinate student/family referral to such services. The BHT is a core member of the collaborative team, including the patient's medical provider and the larger primary care team. Education and Experience:

* If full or temporary approval from MDE is not in place a waiver can be submitted to MDE allowing the social worker to enroll in the necessary classes to obtain the SSW-310.
* With the waiver in place, GRPS has agreed to waive the requirements under Article 14, Section J.1.a which requires two years of employment at GRPS before a GREA member is eligible for tuition reimbursement. This waiver only applies to the classes required to obtain the SSW-310 certification.
* Candidates that access tuition reimbursement will also be required to sign an agreement indicating they will remain employed by GRPS for three years after obtaining their SSW 310 certification. Reimbursement will be required if the candidate does not fulfill this requirement.
