What you will do:
Provide 1:1 Care - No double booking
Demonstrates the knowledge and skills necessary to assess, treat, and care for patients in assigned area. Implements assessment/treatment/care based on the developmental stages of the patient. Interprets verbal and nonverbal communication of the patient.
Evaluates patient?s functional abilities/disabilities utilizing techniques appropriate to the patient type including assessment of strength, range of motion, pain, sensation, balance, transfers, ambulation status, wound size and condition.
In accordance with patient evaluations, establishes treatment plan which specifies treatment goals and treatment modalities that will assist the patient to regain function, increase strength and decrease pain.

Prepares and completes all patient records, billing and documentation according to departmental guidelines.

Utilizes correct body mechanics. Adheres to hospital and departmental safety precautions and infection control procedures in order to maintain a safe patient care and working environment.
Assists with planning and conducting in-service programs and participates in staff meetings, rehab conferences and educational programs. Attends all mandatory in-services.
Assumes responsibility for own professional growth; keeps abreast of developments in the field of Physical Therapy and incorporates approved developments in treatment and evaluation plans.
Participates in departmental Quality Assurance Program and special assignments as requested.
Adheres to all Northwest Community Hospital standards, policies and procedures.

What you will need:
The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
Current Illinois licensure or eligibility.

Adhere to all mandated reporters and OIG guidelines at all times
Complete goal sheets for all students receiving direct and/or consulting services at least one week prior to IEP/FIE
In collaboration with the classroom team, arrange for and perform activities, materials, and prompting hierarchies that relate to the IEP goals throughout the school year
Provide and document directly/consult therapy minutes as required on student IEPs on caseload throughout the school year
Interact with the School Administrator/Program Coordinator/Clinical Coordinator and classroom staff regarding recommendations throughout the school year
Participate in all classroom team meetings on caseload throughout the school year
Participate in at least 2 outside training per school year directly related to serving students with special needs, particularly those diagnosed with autism

You will provide emotional and administrative support to clients in either individual or group settings. Candidates with behavioral therapy experience preferred. Must have Illinois license. Supervision for clinical hours available.
Responsibilities:
Assess nature of clients' situations by interviewing and reviewing personal history
Develop and execute individual treatment plans

About EB Pediatric Resources EB Pediatric Resources is located in Chicago, IL. The Learning Through Play™ Center for Child Development in the Lakeview area offers Playgroups and Classes for children ages 2-5 years. These classes were designed by developmental therapists and early learning specialists to help children transition effectively to preschool and kindergarten. They are small (6-7 children max), provide a teacher/child ratio of one adult for every 2 children, and are taught by experienced therapists. The classes are formatted to provide a playful but structured forum for children to learn socialization and language skills, to separate from their parents in a gentle and positive way, and learn to be comfortable and confident in a structured classroom setting. The classes are particularly helpful for children who may still be learning language skills or have difficulty socializing with their peers.
The Center also provides a pleasant, environment where parents can socialize and browse through books and parent resources. Therapists are available to consult with parents, as needed, and discuss their concerns and goals for their children.
