Responsibilities
Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
Creates plan for care across the continuum, integrating patient/family preferences and values.
Monitor patient care through assessments, evaluations and/or patient records.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
Requirements
Licensure / Certification / Registration:
* Clinical Counselor preferred.
* SocialWorker specializing in Clinical Practice credentialed from the Florida Board of ClinicalSocial Work, Marriage and Family Therapy and Mental Health Counseling preferred.
Education:
* High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Clinical social worker job in Jacksonville, FL
The Mental Health Professional provides girl-centered, trauma-informed counseling and support services to girls and young women impacted by systemic inequities, trauma, and adversity. This role is central to advancing the organization's mission of promoting healing, empowerment, and social justice for girls. The Mental Health Professional works collaboratively with a multidisciplinary team to ensure services are responsive to the complex needs of girls, particularly those involved in or at risk of involvement in the juvenile justice or child welfare systems.

Essential Functions:
Provide individual, group, and/or family counseling that centers the experiences of girls, using trauma-informed, gender-responsive, and culturally affirming practices.
Conduct comprehensive psychosocial assessments, risk and needs evaluations, and develop individualized treatment and service plans.
Inform research and strategic advocacy agendas by elevating girls' voices and stories to Policy Center leadership.
Advocate for girls' needs alongside and/or on behalf of girls and their families in courts, schools, and other systems.
Create a safe, supportive, and empowering therapeutic environment for girls who have experienced trauma, violence, abuse, exploitation, or systemic harm.
Maintain accurate, timely documentation in accordance with clinical, legal, and organizational standards.
Collaborate with care managers, community partners, and family members to coordinate holistic, wraparound services.
Facilitate psychoeducational groups, young women, girls and their families addressing topics such as emotional regulation, healthy relationships, identity, and healing.
Provide crisis intervention and safety planning when needed.
Travel throughout Duval, Clay, Baker, Nassau, and St. John's Counties to provide services in homes, schools, detention facilities, community agencies, and other locations as needed
Participate in clinical supervision, case consultations, and ongoing training focused on girl-responsive, trauma-informed care.
Support program evaluation and outcome tracking efforts through data collection and reporting.
Contribute to creating a culture of respect, inclusion, and accountability within the organization and broader community.

Required Education
Master's Degree in Social Work, Mental Health Counseling, or related field
Individual must be licensed or license-eligible in the state of Florida as a Mental Health Counselor or ClinicalSocialWorker
Experience
Minimum of 2 years working with girls
Professional experience in education, child welfare, social service, and/or juvenile justice
Supervisory Responsibility
This position may supervise volunteer interns.
