Responsibilities

Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations.

Works both independently and collaboratively as part of a multi-disciplinary team in Palliative Care.

Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental health status, legal and safety considerations, brief counseling, transition planning, crisis management and planning and coordination of both internal and external services.

Services may also include assessment and intervention strategies to address behavioral health and addiction issues, as well as end-of-life care and informed decision making about therapeutic options and communication with surrogates, family members, caregivers and health care providers.

May also provide education and facilitate and/or lead support or treatment groups. Throughout the care continuum, social workers will aim to enhance the capacity of people to address their own needs.

Qualifications

Education:

Master of Social Work degree from an accredited School of Social Work required.

Experience:

This position requires a minimum of one-year relevant experience. Experience in one or more of the following areas strongly preferred:

Care Management

Behavioral Health

Social Services

Addictions treatment

Skills:

Broad knowledge of the principles of social work and the ability to apply them, integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic and terminal illness.

Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.

Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.

Demonstrated strong organizational skills, including an ability to effectively triage patient and family needs.

Ability to address the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.

Ability to engage in patient/family discussions of health care goals and decisions with attention to cultural and health literacy implications.

Demonstrated problem solving skills and the ability to deal effectively with complex situations.

Ability to consider the differential influences of the environment, society and culture and develop cultural competence in practice that promotes inclusiveness in the work environment.

Demonstrated strength in verbal and written communication for effective and appropriate interactions with patients, families, physicians and staff.

Position Summary

To promote the psychological well-being of the Columbia student body by serving as the principal liaison to various partner agencies with which CPS contracts to provide afterhours crisis intervention and telehealth clinical services, ensuring the appropriate utilization of these services, and overseeing follow-up with students treated by these agencies.Contributes to a range of other quality assurance activities.Provides direct psychological services to the Columbia student community.

Responsibilities

Direct clinical services ( 45%)
Conduct Treatment Planning sessions. Conduct initial psychological evaluations involving diagnostic assessment, based on a review of presenting problem(s) and of the patient's developmental, social, psychological, and treatment history. Provide solution-oriented, brief counseling to students. Conduct focused support groups including, but not limited to, skills-based groups and groups supporting students in their various identities. Facilitate oversight of outside agencies providing afterhours crisis intervention and telehealth clinical services. Working closely with the Associate Vice President, (CPS), serve as primary liaison to all outside agencies with which CPS contracts for clinical services, troubleshooting students' complaints and concerns, assisting agencies in accessing Columbia resources, and in processing student requests for accommodations. Ensure partner agencies are in compliance with CPS and Columbia policies and procedures and relevant ethical and legal standards. Collect compile, analyze, and report on utilization data from each outside agency With respect to the agency with which CPS has a contract to provide ancillary short-term counseling via telehealth to students: Monitor referrals by CPS providers to ensure they meet established internal criteria for referral Review chart notes of each of the agency's providers for completeness and the clinical appropriateness of the treatment provided. Primary responsibility for authorizing psychiatric services or extended counseling visits based on chart reviews and clinical consultations. Monitor student satisfaction data provided by the vendor; develop and implement internal measures for evaluating vendor's performance, as needed Assist in transitioning students in need of a higher level to appropriate clinical resources
f. Prepare quality assurance reports regarding the performance of all outside vendors. Oversight of aftercare arrangements for students treated by partner agencies and on medical leaves ( 5 %)
Review daily clinical reports from the agency providing afterhours crisis intervention (currently Protocall); working with the Director for CPS Clinical Services, ensure appropriate clinical follow-up with each student contacting the agency Arrange appropriate dispositions and aftercare, as needed, for students seen by outside agencies with which CPS contracts for routine clinical services Provide comprehensive case management to higher risk students or students with complex psychosocial circumstances as they transition through different levels of mental health care which may include hospitalization, residential treatment program, day treatment program, and ongoing outpatient treatment Other quality assurance activities (5%)
Represent CPS at Columbia Health Quality Management meetings; help Identify topics for collaborative QA projects. Participates in other quality assurance activities as assigned Performs administrative duties related to clinical work (5%)
Collaborate with other providers and services within and outside of Columbia Health and to other departmental business. Maintains up-to-date and accurate patient records. Performs other responsibilities as assigned ( 5%)
Minimum Qualifications

MSW in social work New York State LCSW license Minimum of 5 year' related experience required with at least three years of acceptable clinical experience Advanced diagnostic and clinical skills; case management experience, demonstrable experience assessing and managing serious psychopathology (e.g., suicidal and psychotic patients) Superior organizational, interpersonal, and administrative skills Outstanding written and verbal communication skills Demonstrable cultural competence; able to work effectively with diverse staff and students in their various identities Experience in case management Proficiency with electronic health records and related tools
Preferred Qualifications

Background in quality assurance, generating reports Experience supporting inpatients Experience in college or university counseling center or with late adolescent and emerging adult populations Familiarity with local psychiatric resources and social services

Responsibilities:

• Making home visits to clients in designated geographic territories.
• Assist our team in understanding the social and emotional factors related to our clients' health problems.

• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided utilizing a state-of-the-art touch pad tablet.

Qualifications:

• A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
• Must be a Licensed Social Worker.
• Home care experience a plus.
• Ability to work independently and manage time effectively.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases, as assigned.
