Job Description
The Clinical Care Team will take referrals from primary care providers and will work with the primary care team to accomplish the following tasks: Social support navigation for social determinants of health (SDOH) such as food insecurity, housing insecurity, etc.
Compile and maintain a resource list for SDOH resources including eligibility criteria, referral process, and contact information
Collaborate with primary care nurse and providers
Provide in-person or remote social needs screening/assessment with primary care patients referred by nurse or provider
Coordinate or make aware of social services resources, i.e., housing, clothing, food, mental health services, etc.
Collaborate with other socialworkers to identify patient and community resources
Conduct case management activities
Work with hospitals for discharge planning, follow-up and education
Assist with obtaining patient records from hospitals
Assist in securing needed medical equipment through community partners
Conduct follow-up on care plans
Identify patients lost to follow-up or overdue for care and assist them in returning to care
May assist with specialty referral navigation
Schedule, coordinate, and track non-BCS specialist and imaging referrals
Assist with obtaining patient records from specialists and imaging centers
Compile and maintain resource list for specialty referrals including eligibility criteria, referral process, cost and contact information
Assist patients to locate and access low-cost prescription options such as patient assistance programs, discount retailers, etc.
May assist with patient assistance program applications and serve as a patient-provider liaison with the drug companies
Assist patient with applications for programs such as Cover Rx and Rx Outreach
May help with other regional primary care-based initiatives with a social work component
Documents in patients record, updates consults, and tags provider and/or clinical staff as necessary
Provide patient education or find appropriate education resources
Expectations may include:
Complete onboarding and orientation
Participate in regional office and primary care clinical meetings as requested
Attend provider meetings as requested
Attend Health Councils and other community meetings to build relationships with social service agencies and promote health department services
Identify barriers to care or assistance experienced by our patients and seek ways to address them

Qualifications
This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual's ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual's community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.
Job Duties include:
Supervise CST team members to assure the delivery of best and ethical practices
Directs the day-to-day operations of the CST team including scheduling staff work hours to assure appropriate coverage
Provides individual therapy for individuals served by the team
Behavioral interventions such as modeling, behavior modification, behavior rehearsal
Basic Requirements:
Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
Valid driver's license, auto insurance coverage and reliable transportation
Pass criminal background check and other regulatory checks

Education and Experience Required:

We are seeking a Licensed Community Addiction Specialist to join our new Community Support Team (CST). In this role, you will provide therapeutic support for individuals with mental health, behavioral health, and substance use needs, focusing on person-centered, strength-based interventions that promote recovery, increase coping skills, and help individuals achieve their highest level of social functioning.Overview:
We are seeking a compassionate and dedicated Licensed Clinical Addiction Specialist (LCAS), LCAS-Associate, or Certified Alcohol and Drug Counselor (CADC) to join our Community Support Team (CST). In this role, you will provide therapeutic support for individuals with substance use and co-occurring mental health needs, focusing on person-centered, strength-based interventions that promote recovery, symptom reduction, and improved coping skills.
Pay: $60,000
Sign on Bonus $2000
Key Responsibilities:
Conduct clinical assessments to identify needs and guide treatment planning
Provide individual, family, and group therapy sessions in outpatient and inpatient settings
Develop and implement person-centered treatment plans
Coordinate care with interdisciplinary team members including psychiatry, nursing, behavioral health specialists, and case managers
Collaborate with community partners, legal systems, families, and social service agencies to ensure comprehensive support
Facilitate the orientation of new service users to RHA programs
Oversee discharge planning to support continuity of care and community reintegration
Serve as a positive, engaged member of the clinical team, providing backup to the program director or other therapists as needed
Requirements:
Active LCAS, LCAS-Associate, or CADC certification
Graduate degree in Social Work or a related human services field
Minimum 2 years of experience working with individuals with mental health and/or substance use disorders, including developing treatment plans, providing therapy, and making mental health diagnoses
Provisional licensure in North Carolina is acceptable
Working knowledge of the human services delivery system in North Carolina
Valid driver's license, auto insurance, and reliable transportation
Ability to pass a drug screen and background check
Preferred Qualifications:
Employee supervisory experience
Experience with Electronic Medical Records (EMR)
Skills and Attributes:
Excellent communication and listening skills
Strong therapeutic and behavioral interviewing skills
Ability to work collaboratively within an interdisciplinary team
Capacity to anticipate client needs and provide effective interventions
Physical Requirements:
Ability to lift up to 40 lbs., pull up to 25 lbs., and perform physical tasks such as squatting, kneeling, crawling, crouching, climbing, and stooping
Ability to stand and walk regularly, and use hands for various tasks
Ability to perform CPR from the floor level, requiring kneeling, bending, and lifting
Visual requirements include close, distance, and peripheral vision
Must be able to talk and hear
