What you will do:
Provide 1:1 Care - No double booking
Demonstrates the knowledge and skills necessary to assess, treat, and care for patients in assigned area. Implements assessment/treatment/care based on the developmental stages of the patient. Interprets verbal and nonverbal communication of the patient.
Evaluates patient?s functional abilities/disabilities utilizing techniques appropriate to the patient type including assessment of strength, range of motion, pain, sensation, balance, transfers, ambulation status, wound size and condition.
In accordance with patient evaluations, establishes treatment plan which specifies treatment goals and treatment modalities that will assist the patient to regain function, increase strength and decrease pain.
Administers and instructs patients in performance of all aspects of the treatment plan, assessing the effectiveness and modifying as necessary.
Prepares and completes all patient records, billing and documentation according to departmental guidelines.
Communicates and works in cooperation with physicians, nursing and other staff to provide optimal patient care.
Involves the patient, family and caregivers in the plan of care, including next-level-of-care planning. Conducts patient/family education sessions and provides self-directed programs considering the developmental needs of the patient.
Assists with supervision of paraprofessional staff, students and volunteers as directed; delegates tasks and monitors performance of tasks to ensure they are completed correctly.
Utilizes correct body mechanics. Adheres to hospital and departmental safety precautions and infection control procedures in order to maintain a safe patient care and working environment.
Assists with planning and conducting in-service programs and participates in staff meetings, rehab conferences and educational programs. Attends all mandatory in-services.
Assumes responsibility for own professional growth; keeps abreast of developments in the field of Physical Therapy and incorporates approved developments in treatment and evaluation plans.
Participates in departmental Quality Assurance Program and special assignments as requested.
Adheres to all Northwest Community Hospital standards, policies and procedures.

What you will need:
The completion of a degree in Physical Therapy from an A.P.T.A. accredited school, or the equivalent.
Current Illinois licensure or eligibility.
CPR certification through the American Heart Association.

This position is in the Adult Protective Services Unit. APS is a unit within the Aging and Disability Resource Center of Waukesha County Health and Human Services. Waukesha County Adult Protective Services works with adults (18 years and older) with disabilities and elder adults (60 and older) in Waukesha County who are at risk and/or experiencing some form of abuse, neglect, or financial exploitation. The primary role and responsibility of an Adult Protective Services SocialWorker is to investigate and intervene in reports of abuse, neglect, and financial exploitation as guided by WI State Statutes (46.90, Ch 54, and Ch 55). The goal is to link persons at risk, or currently experiencing, abuse and neglect with protective services in the least restrictive means to promote their best possible quality of life. Socialworkers work with adult clients across the lifespan that present with a variety of risks to their health, safety and well-being to determine the most appropriate course of intervention to meet their individual needs. Every action taken by Adult Protective Services must balance the duty to protect the safety of a vulnerable adult with respecting a competent adult's right to self-determination. Adult protective services socialworkers work closely with the court to provide legal protection of incompetent adults through a variety of court interventions including court ordered guardianships and protective placements into residential facilities.
Strong candidates for the open position will meet minimum qualifications for the position and have for following knowledge and/or experience:
* Direct work with vulnerable adult populations including adults with developmental disability, adults with chronic mental health and/or substance abuse concerns; older or frail elderly; and persons with dementia.
* Case management and in-home assessments to vulnerable adults in the community.
* Court work, documentation, and testimony - with preference to those with experience and or knowledge of Wisconsin Stats 51, 54, 55, and 46.90.
* Skill in working with involuntary clients and/or clients with complex needs.
* Knowledge of social work skills for assessment, case planning, and documentation.
* Professional skills in communicating and collaborating with a variety stakeholders including law enforcement, court personnel, human services professionals and other community stakeholders.

For Candidates hired with a SocialWorker Training Certificate (-127) the progression on years of pay will start when a full SocialWorker credential is issued (of any of the other types allowed below).
Pay will be set in accordance with the Compensation Plan and Wisconsin Administrative Code in effect at the time of hire. A probationary period is required.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
To receive the bonus, a candidate must sign an agreement indicating they will remain in that occupational area for a year. If the candidate leaves their occupational area prior to completing the one year, they may be required to reimburse the initial payment.

The ideal candidate will conduct comprehensive psychological assessments, develop detailed evaluation reports, and help guide clients toward appropriate care and resources. 4. Facilitate the effective utilization of existing community resources to meet the needs of school children and shall assist in developing services meetings as necessary.
5. Attend regular building special service meetings as necessary.
6. Participate in annual reviews for special education students as necessary.
7. Conduct building in-service workshops for staff and parents.
8. Participate in district in-service workshops.
9. Offer individual and group counseling service.
10. Facilitate groups of at-risk students regarding anger management techniques, bullying/harassment issues within the high school setting.
11. Coordinate/disseminate information to staff and students regarding drug and alcohol issues within the school environment.
12. Participate in RtI/Problem Solving meetings as appropriate.
13. Perform other tasks and responsibilities assigned by the supervisor.
