Tasks and Responsibilities:
Practice within the guidelines of their state licensing board, American Psychological Association Ethical Principles and Code of Conduct, and MTF privileges.
Conduct psychological evaluations utilizing information from clinical interviews, psychological testing, and collateral sources, as appropriate.
Establish psychiatric diagnoses according to the American Psychiatric Association Diagnostic and Statistical Manual of Disorders.
Provide psychological treatment for individuals and groups; help medical and surgical patients deal with illnesses or injuries.
Assist in rehabilitation of patients.
Deliver brief, consultation -based services to patients and PCMs using an integrated care model. Services provided are standard and mimic the structure, function, and pace of primary care (i.e., psychotherapy or other specialty care psychological assessments are inappropriate and therefore prohibited). The PCM is ultimately responsible for the patient's health care plan and makes the determination on how the HCW's recommendations might be integrated into the PCM's plan for the patient.
Provide timely and succinct feedback to primary care physicians regarding consultations findings and recommendations.
Determine the appropriateness of the patient receiving services in the primary care setting and assisting with implementation of treatment plans.
Formulate behavioral health interventions appropriate to primary care setting and assisting with implementation of treatment plans.
Provide brief follow ups, including relapse -prevention education.
Develop, teach, and provide oversight for classes that promote education and skill-building to enhance psychological and physical health.
Provide concise documentation of care and recommendations in patient's medical record in no less than 24 hours after the patient is seen.
Participate in group medical appointments including Shared Medical Appointments (SMA) and Drop-in Group Medical Appointments (DIGMA) as requested by the PCM.
Provide technical direction and clinical insight into relevant projects being conducted at the site of service.

Responsibilities
Work with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as, history of trauma, co-occurring conditions and suicide.
Assist clients in identifying and managing personal, psychological and interactive problems, and in achieving effective personal, occupational, marital, and family development and adjustments.
Provide comprehensive social work services in the form of casework and case management by consulting on or developing discharge plans for complicated cases, psychosocial assessments and mental status examinations on behalf of beneficiaries.
Analyze information collected to identify symptoms, determine psychiatric diagnosis, develop treatment plan and/or determine advisability of counseling or referral to other specialists or institutions.
Conditions of Employment:
Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Secret security clearance or access for the duration of employment. A background investigation and credit check are required.
This position requires Basic Life Support training certification from, or in accordance with, the American Heart Association and American Red Cross. Current Advanced Life Support or other advanced certification does not supersede BLS completion.

Responsibilities
* Work with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as, history of trauma, co-occurring conditions and suicide.
* Assist clients in identifying and managing personal, psychological and interactive problems, and in achieving effective personal, occupational, marital, and family development and adjustments.
* Provide comprehensive social work services in the form of casework and case management by consulting on or developing discharge plans for complicated cases, psychosocial assessments and mental status examinations on behalf of beneficiaries.
* Analyze information collected to identify symptoms, determine psychiatric diagnosis, develop treatment plan and/or determine advisability of counseling or referral to other specialists or institutions.
* Conditions of Employment:
* Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
* This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Secret security clearance or access for the duration of employment. A background investigation and credit check are required.
* This position requires Basic Life Support training certification from, or in accordance with, the American Heart Association and American Red Cross. Current Advanced Life Support or other advanced certification does not supersede BLS completion.
* This position requires that applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.

The Edgemoor psychologist position is a full-time, on-site position serving 192 residents in the distinct part skilled nursing facility. Job duties are varied and depend on the strengths and interests of the applicant but include individual psychotherapy, capacity evaluation, Behavioral Care planning, interdisciplinary collaboration, group therapy, and integration with other disciplines to provide a therapeutic milieu for patients that have complex medical and psychiatric and psychosocial conditions. Ability to do neuropsychological testing is a plus. Will participate also in setting and writing policies and procedures, chairing the Quality of Life Committee, attending the Quality Council Committee, and supervising the Social Work Department which consists of 5 staff members.
The ideal candidate will have experience working with culturally diverse youth in a fast-paced, dynamic environment such as juvenile detention facilities, acute psychiatric facilities, or residential treatment facilities. Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (******************** is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
Under the direction of the Center Director, is responsible for direct social work case management services to participants.
Essential Functions:
Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants' baseline status to appropriate staff on a daily basis.
Involved in the development and implementation of QAPI activities.
In conjunction with the IDT, may conduct the initial intake, meeting with family members and others. Coordinate ongoing family meetings, as needed.
Obtain extensive social history from participant and/or family member upon admission to program.
Provide individual and family counseling as needed or prescribed in the plan of care; develop and lead group counseling and support activities.
Provide crisis intervention and advocacy as required.
In conjunction with the interdisciplinary team, coordinate discharge planning for participants returning home from hospital or nursing facility.
Maintain current, written case management records, including ongoing documentation of services provided, reassessment of changing needs and participant's expressed wishes.
Act as liaison between the participant and other agencies such as Department of Aging, Social Security Administration, Medicaid, etc.
Assist with money management.
Assist with ongoing financial eligibility for participants, including recertification as needed.
Participate in participant-related conferences in the community as designated.
Maintain confidentiality of participant information.
Non-Essential Job Functions:
Attend and participate in all staff meetings, in-services, projects, and committees as assigned.
Adhere to and support the center's policies, practices, and procedures.
Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
Be flexible in schedule of hours worked.
Will require use of personal vehicle.
Education and Certification:
Master's degree in social work required.
Original valid driver's license with proof of auto insurance.
Bilingual English/Spanish preferred.
Skills and Experience:
Minimum of one (1) year of documented experience in working with a frail or elderly population required.
Prefer experience in a community-based setting or geriatric program.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
