**Job Description:**
Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate. Assesses patients' and families' wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning. Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan. Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs. Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate. Actively participates in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for all assigned patients to identify resources necessary at discharge and ensure a timely transition, escalating care delays to leadership as appropriate. Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness and mobilizes family/community resources to meet identified needs while advocating for patient and family empowerment in making health care decisions and accessing needed services. Organizes and facilitates patient and family care conferences with the multidisciplinary team. Documents discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. Provides patient and family advocacy, and support patient's choice and patient rights during hospitalization. Communicates with Payors patient's needs for authorization for post-acute care as needed. Other duties as assigned. Assesses readmitted patients for the patient's and family's perceived reasons for the readmission.

The CSU Pool Therapist's purpose is to provide individual, family, and group therapy to children/youth who are admitted to the Crisis Stabilization Unit. The CSU's mission is to use an interdisciplinary team of clinicians, residential counselors, medical staff and admissions staff to provide brief, 3-5 day, crisis stabilization treatment for children and youth with behavioral health issues, including depression, suicidal thoughts, other trauma reactions, and family issues. Our team coordinates treatment and provides tools and safety planning for children/youth and families so that they can safely return to their homes and the community. We are committed to serving Colorado's children/youth while maintaining a non-judgmental and inclusive approach.
Essential Duties/Responsibilities:
Meets with all assigned children/youth, conduct clinical interviews and set up initial staffing in order to develop treatment plans for each individual.
Maintains caseload as assigned by Manager or Program Coordinator by overseeing the implementation and review of all treatment services provided to each child/youth on caseload.
Primary coordinator for discharge planning and transition of care to community resources for each child/youth on caseload

Colorado Legal Services (CLS) is a statewide nonprofit legal aid program, with thirteen offices providing civil legal assistance to low-income Coloradans throughout the state. The Farm Worker Rights Division (FWRD) of Colorado Legal Services represents farm workers in federal and state courts, and administrative agencies on a variety of civil legal claims. The FWRD's work focuses primarily on wage theft, human trafficking, immigration, sexual harassment, working conditions and civil rights. The FWRD also runs a comprehensive community education program that involves outreach to workers at labor camps and to community-based groups. The FWRD's client community is comprised primarily of Latino migrant and seasonal farm workers.
The FWRD currently has four summer internship positions for students who share a passion for advocating for the rights of low-wage farm workers. The interns will be based in the regions with the largest population of migrant and seasonal farm workers to conduct outreach to farm workers at labor camps and other locations. The interns will work with clients and potential clients on a variety of legal issues and will travel throughout the rural areas of their respective regions to visit farm workers and conduct outreach at labor camps, collaborate with key partners in the region, and ensure workers have information about their rights and the services that the FWRD provides. When not out in the field, the interns will have the opportunity to utilize their legal research and writing skills to support the attorneys with ongoing federal litigation involving claims arising under the Trafficking Victims Protection Act, the Federal Labor Standards Act, the Racketeer Influenced and Corrupt Organizations Act, Colorado wage laws and more.
Location One intern will be placed in each of the following regions of Colorado:
Northeastern (NE) Region - the NE intern will work primarily out of the CLS Greeley office with travel to Brighton-Greeley-Fort Morgan-Yuma areas to conduct outreach.
