You will also complete Disability Benefits Questionnaires (DBQs) through the Web Portal.
The Licensed Psychologist DOES NOT make disability determinations. The U.S. Veterans Administration makes all disability determinations, in part using the findings recorded in the DBQs.
This role ONLY performs Disability Assessments on Veterans. You will NOT treat, and you will NOT prescribe medication in this position.

Essential Functions
Conducts psycho-social assessments on each patient and determines social work treatment plan compatible with a total medical treatment plan per CMS guidelines (initial, 90-day, annual and as needed for unstable patients). Track CIPA/POC for SNFs, coordinate POC meetings with RN, RD, MD and patient.
Complete KDQOL and other tools as required
Provide monthly documentation in every patients chart on caseload.
Provide education to new patients regarding their Rights and Responsibilities as well as the Grievance Procedure as well as documentation that this was completed.
Complete monthly tracking of patients needed for QAPI.
Maintains professional working relationship with other staff of Concerto as well as in the skilled nursing facility. Participates in administrative and staff programs as required, including attending staff meetings and in-service meetings.
Compiles and develops educational materials to be used as teaching aids in education of patients and staff as needed.
