We are seeking either a Texas licensed LPC or LCSW to join our behavioral health care management team as a Transition of Care Coach, supporting our Medicaid members with a behavioral health diagnosis and have been recently admitted to an inpatient unit. You will partner with the hospital's discharge team from admission to discharge to make sure the transition from inpatient is successful. Preferred candidates will have previous discharge planning or case management within another MCO or an inpatient program.
Excellent computer skills and attention to detail are very important to multitask between systems, talk with members on the phone, and enter accurate contact notes. Mileage is reimbursed through our benefit package.
Job Summary
Provides support for care transition activities. Facilitates transitional care processes and coordination for member discharge from hospital admission to all other settings. Strives to ensure that best possible services are available to members at time of hospital discharge, and focuses on goal to reduce member readmissions. Contributes to overarching strategy to provide quality and cost-effective member care.
Essential Job Duties
- Follows member throughout a 30 day program that starts at hospital admission and continues oversight through transitions from acute setting to all other settings, including nursing facility placement/private home, with the goal of reduced readmissions.
- Ensures safe and appropriate transitions by collaborating with the hospital discharge planner, as well as collaborating with hospitalists, outpatient providers, facility staff, and family/support network.
- Ensures member transitions to setting with adequate caregiving and functional support, as well as medical and medication oversight support.
- Works with participating ancillary providers, public agencies or other service providers to make sure necessary services and equipment are in place for safe transition.
- Conducts face-to-face visits of all members while in the hospital and, home visits high-risk members post-discharge as needed.
- Coordinates care and reassesses member needs using the Coleman Care Transition model post-discharge.
- Educates and supports member focusing on seven primary areas (Transition of Care Pillars): medication management, use of personal health record, follow-up care, signs and symptoms of worsening condition, nutrition, functional needs and or home and community-based services, and advance directives.
- Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.
- Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
- Facilitates interdisciplinary care team meetings (ICT) and collaboration.
- Provides consultation, recommendations and education as appropriate to non-behavioral health care managers.
- 40-50% local travel may be required (based upon state/contractual requirements).
Required Qualifications
- At least 2 years experience in health care, with at least 1 year of experience in hospital discharge planning, care management or behavioral health setting, or equivalent combination of relevant education and experience.
- Licensed behavioral health clinician to include: Licensed ClinicalSocialWorker (LCSW) or Licensed Professional Counselor (LPC), License must be active and unrestricted in state of practice.
- Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements, unless otherwise required by law.
- Knowledge of or experience using the Care Transitions Intervention (CTI) or similar model.
- Background in discharge planning and/or home health.
- Demonstrated knowledge of community resources.
- Proactive and detail-oriented.
- Ability to work within a variety of settings and adjust style as needed - working with diverse populations, various personalities and personal situations.
- Ability to work independently, with minimal supervision and demonstrate self-motivation.

Your Mission in Action
The Clinician is an integral member of the program's clinical team, responsible for providing individual, group, and family therapy to children served. The clinician conducts comprehensive assessments, collaborates with multidisciplinary treatment teams, and develops and implements individualized treatment plans. Monitor the child's emotional well-being, provide individual counseling at least once a week, assess their adjustment to shelter living, and implement crisis interventions and safety plans when necessary
Use positive behavior management techniques, including active listening, de-escalation, and problem-solving, while tracking behavior to modify interventions and reduce the need for physical containment
Collaborate with children to establish treatment goals and regularly update treatment plans, creating safety plans as needed for specific child accommodations
Coordinate, schedule, and facilitate individual, family, and group therapy sessions to address the child's emotional, mental, and social needs
Maintain and update case files, documenting progress, behavioral challenges, and incidents, and ensure timely reporting of abuse, trafficking concerns, and session progress notes
Provide input on reunification, assess sponsors' emotional support, develop discharge safety plans, and recommend post-release services to ensure continued care
Work with clinical staff, multidisciplinary teams, and community partners, participating in weekly meetings to coordinate efforts for successful reunification and program goals
Foster positive relationships with stakeholders, demonstrate cultural awareness and sensitivity, and ensure compliance with policies and regulations
Participate in emergency evacuation procedures, either assisting with evacuations or remaining on-site to ensure safety during natural disasters or emergencies

The duties include, but are not limited to the following:
The SNC socialworker oversees and manages the installation Exceptional Family Member Program - Medical (EFMP-M) IAW DoD and AF policy and any subsequent implementing guidance.
Uses clinical assessment skills and maintains procedures to identify sponsors whose family members have special medical and educational needs in a timely manner. The SNC uses a multi-disciplinary and collaborative approach with other key service providers, such as installation youth and childcare facilities, and officer and enlisted spouses groups, to ensure effective outreach and identification of special needs. Ensures family members of the AF personnel or family of the AF member are appropriately "Q coded" (EFMP- enrolled).
Ensures all active duty sponsors known to the Air Force Personnel Center, and local Military Personnel Section (MPS) and/or Commander Support Staff (CSS) where applicable, having family members with special needs are identified in AF medical special needs data management systems. Provides oversight for base-level data entry in AF-provided data management systems used in the management of EFMP-M.
Provides training to installation personnel and medical staff; and consultation as needed to support the implementation of EFMP-M base-wide.
Integrally involved in the Family Member Relocation Clearance (FMRC) process. Ensures all FMRC requirements are implemented by all EFMP-M staff. Coordinates the enrollment process and travel screening for all active duty family members' and travel screening upon request to the families of DoD civilian sponsored assignments going to an overseas base.
Provides oversight to ensure every AF sponsor with one or more family members with special needs assigned to the installation has a SN file maintained at the Medical Treatment Facility (MTF). Ensures Q-coded sponsors assigned to the installation are contacted annually to determine if there are unmet needs and to request updates of information as needed.
Collaborates with the Integrated Delivery System (IDS) and other interagency forums that provide family medical information and referrals to base and civilian agencies. Ensures EFMP families are referred to the Airman and Family Readiness Center for additional community assistance as needed. Maintains a cooperative working relationship with the base MPS, CSS, and with Air Force Personnel Center (AFPC). Actively supports the integration of EFMP-M, Exceptional Family Member Program - Family Support (EFMP-FS) and Exceptional Family Member Program - Assignments (EFMP-A) services at the installation. Provides information, appropriate contact information, and coordinates referrals as appropriate.
Participates in EFMP quarterly case reviews to discuss/assess newly identified families', complex, or unmet medical needs. Determine the appropriate resources necessary for the families.
Patient specific assessments and clinical plans of action will be located in AHLTA and the Special Needs record. Meeting minutes will not contain patient identifying information.
Coordinates and participates in designated facility meetings.
Participates in the clinic orientation and training of other staff of the Special Needs Program. May serve on committees, work groups, and task forces at the facility.
Must maintain a level of productivity and quality consistent with: complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional social work, Health Services Inspection (HSI); and other applicable DoD and service specific guidance's and policies. Must also comply with the Equal Employment Opportunity (EEO) Program, infection control and safety policies and procedures.
Completes medical record documentation and coding and designated tracking logs and data reporting as required.
Completes all required electronic medical record training, MTF-specific orientation and SNC training programs.
Consults with medical personnel, legal authorities, military commanders and school districts, as required.
Attends and participates in meetings during normal duty hours and professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, commander's staff meetings, primary care staff meetings, and others required by applicable regulations, MTF guidance, or as directed by department leadership.

Develops, implements and maintains a successful student assistance service program to reduce risk factors in children's learning in collaboration with the Supervisor of Social Services and district-wide personnel.

Enables families to convey their concerns to school personnel and maintains open lines of communication.
Works in collaboration with parental involvement programs in pursuit of avenues which will enable parents to be more involved with their child's school program and school activities.

Works cooperatively with all departments in regards to students' needs.
Collaborates with counselors and teachers in meeting the needs of students.
Provides counseling resources and materials to campus administrators, counselors, and teachers within the limits of available resources.
