Collaborates with the patient, family and multidisciplinary team to assure psychosocial needs are addressed. Utilizes review of medical record, interview of patient/family and consultation with the interdisciplinary health care team to complete screenings/assessments/plan of care. 2. Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability and other psychosocial issues identified. Provides crisis intervention and counseling for patients and families. Promotes patient and family empowerment and independence. 3. Participates in discharge planning activities. Implements psychosocial portion of plan to facilitate timely patient discharge and promote optimal utilization of patient and community resources. Provides planning for post-hospital placements including Home, Nursing Home, Rehabilitation, Shelters, Home Care, Hospice and DME requirements for safe discharge. Accesses and mobilizes family/community resources to meet identified needs. Intervenes with families with complex psychosocial needs and family dynamics. Educates patient/family and physician about post-acute options and addresses issues of choice. 4. Communicates daily with interdisciplinary team regarding status of discharge planning activities. Validates discharge criteria/plans for patients and families and notifies the interdisciplinary team of newly identified resources or changes in previously identified plans/resources. 5. Provides intervention in cases involving child or adult abuse/neglect (serves as liaison between Parkland and CPS/APS), domestic violence, guardianship (temporary/ permanent), foster care, adoption, mental health level II placement, and sexual assault. 6. Documents screening, assessment, interventions and outcomes to ensue effective communication per established documentation procedures. 7. Manages referrals and communications from a variety of sources such as fax referrals, calls, physicians, COPC Social Workers, Care Management Team, the health care team, community agencies, patients, families, etc. to ensure optimal customer service. Utilizes and/or refers to Patient Support Center as appropriate. 8. Maintains and implements knowledge of all rules, regulations, policies, procedures, laws and guidelines, including The Joint Commission, that impact the Social Work field and ensures that patient discharge plans are in compliance. 9. Maintains positive working relationship with management, vendors, other hospitals, employees, and community agencies to ensure effective communication among all patient contacts. Keeps staff informed, makes reports, rounds and frequent agency contacts to ensure timely discharge to appropriate setting and support. 10. Reports assigned data (i.e. services provided, Medicare stats, alcohol and drug numbers, adoption numbers, CPS/APS cases) as needed, ensuring timeliness and accuracy. 11. Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the department and Parkland. 12. Stays abreast of the latest developments, advancements and trends in the field of Social Work by attending seminars or workshops, reading professional journals, actively participating in professional organizations and maintaining licensure in Social Work. Training and expertise in evidenced-based psychotherapies preferred.
Experience across a range of modalities beyond individual adult treatment to include child, couples, family, and group treatment is preferred.
Must have experience treating and diagnosing posttraumatic stress as well as comorbid diagnoses (e.g., anxiety), with evidence-based treatments and providing clinical supervision in these areas.
Experience working with a military or veteran population is preferred.
3+ years of clinical experience is preferred.
Experience utilizing an electronic health record is required.

ESSENTIAL FUNCTIONS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential duties.
Maintains caseload of students/families referred for additional support
Provides brief assessments to develop an action plan with students and families
Facilitates family intervention meetings with families in crisis or with multi-dimensional needs
Serves as point-of-contact for teachers and administration for families in crisis; supports staff training and implementation of support processes
Serves as liaison for ESSA subgroups, McKinney-Vento, and migrant students; follows up with families by phone or in-person, coordinates with external agencies (e.g. Child Protective Services, The Department of Juvenile Justice, community service agencies, etc.) as needed
Maintains an active role and awareness of local community resources within school boundaries, through research, attending local meetings and/or networking events.
MINIMUM REQUIRED QUALIFICATIONS:
Master's degree in social work or related social service field AND
One (1) year of documented social work field experience AND
Social work education course work, to include Human Behavior in the Social Environment equivalent OR
Equivalent combination of education and experience
Ability to clear required background check
OTHER REQUIRED QUALIFICATIONS:
Microsoft Office (Outlook, Word, Excel, PowerPoint, Project, Visio, etc.); Web proficiency.
Ability to travel 20% of the time
Three (3) years of field experience
Background working in education

We are seeking a Part-Time Licensed Social Worker. The Licensed Social Worker plays a key role in supporting the emotional and social well-being of our residents. Under the direction of the Administrator, this role is responsible for planning, organizing, and developing programs and services that meet the medically related emotional and social needs of each resident. The ideal candidate is professional, compassionate, and committed to person-centered care.
Key Responsibilities:
Assess and support the emotional and social needs of residents on an individual basis.
Maintain accurate documentation in resident medical records according to facility policies and federal/state regulations.
Collaborate with interdisciplinary teams to coordinate continuity of care.
Serve as a liaison between residents, families, and community agencies.
Participate in care planning and discharge planning processes.
Other duties as assigned by the Administrator.
Qualifications:
Must be a Licensed Social Worker in good standing.
Completion of secondary education at an accredited school of Social Work.
Prior experience in a supervisory capacity is preferred.
Experience in geriatric, adult, or long-term care social work is strongly desired.
Excellent communication and interpersonal skills with the ability to work professionally and cooperatively with residents, families, and team members at all levels.
