As part of the LTAC unit, Long Term Acute Care Nurses (RN) are responsible for caring for patients who require extended medical care.
Provide care for 5-6 patients.
Adjust and perform in fast paced environment
Keep up with many admissions and discharges
Be able to monitor and administer medications (orally, subcutaneously, and through an IV)
Educate patients and family on surgical processes and how to manage their illness/injury
Monitor and adjust specialized equipment used on patients, and record and interpret electronic displays
Interpret and understand telemetry readings, notifying appropriate medical team members of any irregularities
Perform the duties required of a registered nurse at all times
Care for patients with pulmonary HTN, respiratory failure, COPD, CHF, and A-Fib
Use computer equipment to monitor cardiac and other vital information
Start IV's, draw blood, see the patient from start to finish

The Department's responsibilities include 1) planning and coordination of state/federal investments into programs and services aimed at improving quality of life for senior adults across the region, 2) coordinating services among a network of senior centers and providers, 3) providing counseling/ assessments to help Middle Tennesseans enroll in programs and services, and 4) monitoring the performance of grant-funded programs to ensure compliance with state and federal requirements.
GNRC's CHOICES Intake team helps to screen and assess older adults who may be eligible for the TennCare sponsored CHOICES program funded through Medicaid. CHOICES provides nursing facility services and home and community-based services for adults 21 years of age and older with a physical disability and seniors (age 65 and older).
Responsibilities of this position may include:
Help inform older adults about programs and services available to assist with long-term care, transportation, nutrition, or other needs as appropriate,
Conduct telephone and in-person screenings and assessments with clients across the Middle Tennessee area,
Complete pre-admission evaluations and collect financial and medical history information from potential clients,
Keep informed on aging and disability related issues and gather information about available resources and local service providers,
Help conduct periodic customer satisfaction surveys to assist GNRC and TennCare with monitoring the quality of the process,
Document information within the appropriate systems, and
Other duties as assigned by the CHOICES Intake Program Manager or the Director of Aging and Disability Services.
Qualifications: The ideal candidate would have a bachelor's degree in social work, with an active social work license, or LPN. Must have excellent verbal and written communication skills. Experience in geriatrics and case management strongly preferred. Bilingual Language Skills a Plus. Foreign degrees and credentials will be considered.
Required Technology Skills: Proficiency with Microsoft Office and Adobe Acrobat software is required.
Travel Expectations: This position will require travel throughout GNRC's thirteen county region. A valid driver's license and/or access to personal transportation is required.

The therapist leads the collaboration among an interdisciplinary team to develop, coordinate, and facilitate all direct social services interventions to patients assigned to their caseload and their support systems. The therapist utilizes empirically supported therapeutic techniques, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), or family therapy, to aid in the stabilization and recovery of individuals with mental and behavioral health concerns. Job Duties & Responsibilities:
Conduct individual, group, and family therapy sessions: Providing one-on-one therapy, group therapy, and family therapy sessions to address patients' specific needs and goals.
Assessing patients' needs: Conducting comprehensive assessments to identify patients' mental health concerns, strengths, and challenges.
Developing treatment plans: Creating individualized treatment plans based on patients' needs and goal; provide guidance in the implementation of clinical interventions, including but not limited to facilitating treatment team meetings.
Providing counseling and support: Incorporating evidence-based therapeutic techniques to help patients manage their symptoms and improve their overall well-being within sessions and in-the-moment as needed in the milieu.
Educating patients: Teaching patients about their mental health and/or substance use condition, self-care strategies, and treatment options to support patients in reviewing the costs and benefits of treatment engagement and aftercare plans.
Documenting patient progress: Maintaining detailed records of therapy sessions, including patient progress, goals, and interventions. Complete all documentation within required time.
Collaborating with other professionals (both internally and externally): Working with other members of the treatment team, such as psychiatrists, socialworkers, and case managers, to coordinate care and ensure optimal outcomes.
Adhering to ethical guidelines: Following ethical codes of conduct and maintaining confidentiality in all interactions with patients.

Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The Family Intervention Specialist:
Carries a caseload of 4 to 6 families
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan
Provides on-call crisis support to the youth and family (schedules vary by location)
Completes accurate and timely documentation in an electronic medical record system (EMR)
Performs other duties as assigned

Schedule is flexible and non-traditional as it is based around the availability of youth and families served.
