The Social Worker is an active member of the Hospice Interdisciplinary Team. The Social Worker assumes, in part, responsibility for own personal and professional development. Maintain the standards of Social Work in accordance with the guideline set forth by NJDOH, JCAHO, NHO and NJI-10. Performs a comprehensive psychosocial assessment on all patients (refer to SW P/P) to routinely include the philosophy and services through the CFHH&PC program. -Performs/maintains ongoing follow-up re-assessment visit, to determine ongoing needs of patients/families, as well as determining appropriateness for Hospice Program. Assists with the development and implementation of the interdisciplinary care plan as it relates to the psychosocial needs of the patient/family. Participate in joint visits or family conferences with other disciplines when needed. -Maintains clear and concise documentation of all visits, contacts, and interdisciplinary team case conferences in a timely manner. Maintains necessary documentation on interdisciplinary care plan. Assists with identification and completion of Risk Assessments. Provides individual and family counseling to assist in coping with the effects of loss, grief, death and dying, and bereavement. Facilitates patient/family support and/or educational groups where identified or requested. Provides patient information to departments engaged in statistical collection and related services. Identifies and provides linkage to community resources, acting as an advocate and serving as a liaison between patients and entitlement programs. Assists with the education and completion of Advance Directives in accordance with the New Jersey Advance Directives for Health Care Act and the Federal Patient Self Determination Act. Assists patients/families with information regarding burial as needed. Verifies and obtains copies of patient's Social Security number and insurance. Investigates secondary insurance information with regard to Hospice Care benefits. -Obtains, on initial Social Work Assessment, all required financial information to maximize alternate sources of reimbursement for hospice services and services covered by the patient's insurance plan. Participates in the following meetings: AM MEETINGS IDT MEETINGS TEAM MEETINGS SW STAFF METINGS GENERAL STAFF MEETINGS Participates in subcommittee meetings as requested, i.e. QI, PIP, Forms meetings.
