* Assesses reports of abuse, neglect, or dependency of children using either a forensic investigative assessment approach or a family assessment approach, depending on the severity of the maltreatment reported, and in accordance with state law and divisional policy governing such. Preliminary reviews existing DSS documentation (if such exists) and staff response process with the Supervisor. Conducts a comprehensive assessment including interviews of all victim children, all caretakers, all person living in the home, all appropriate collaterals (as indicated by both the reporter and the family being investigated), and all other persons, entities, agencies, professionals, relatives, etc. who might have knowledge about family relationships, behaviors, and incident (or incidences) that prompted the report to DSS. Works collaboratively with law enforcement and prosecutors on joint investigations of a forensic nature. Follows mandated timeframes and procedures for appropriate initiation and continuous process of assessment. Observes the home structure and environment, ensuring that persons, provisions, and location are able to meet the basic needs of each child involved in the report. Completes safety assessments (initial and final) and all structured documentation tools with the family to promote a transparent process. When necessary, explore alternative safety resource placement options with the family to avoid removal of physical or legal custody. Engages the family in planning for safety and well-being, encouraging immediate action and front-loading services. Incites the professional opinions of necessary contributors (law enforcement, medical personnel, mental health professionals, etc.) in order to determine what safety measures are necessary adequately.
* Invokes court action when children cannot be safe in their homes, seeking direction through legal consultation, and completion/filing of all necessary court paperwork. Accompanies law enforcement in taking physical custody of child(ren). Collaborates with internal licensing/placement experts to identify and locate placement that best meets the child's needs and physically delivers the child to the caretaker.
* Participates in forensic joint interviews at the local Child Advocacy Center on appropriate cases (with law enforcement and any other appropriate professionals).
* Participates weekly in unit/team staffing, presenting completed case assessments for unit/team decision-making. Solicits guidance, direction, and support from the supervisor individually at any point in case and/or requests assistance in completing tasks, if needed.
* Documents all activities, contacts, dates, and times on a structured documentation tool in NC Fast. Requests necessary supporting documentation from external sources (i.e., medical/hospital records, police reports, 911 response sheet) and places such in the case file.
* Participates in local, professionally-enhancing, and state-mandated trainings. Participates in monthly MDT meetings to discuss specific cases in a multi-disciplinary setting. The care manager works to coordinate services within the community to support the child's developmental, social, emotional, and physical well-being. The services coordinated through this program include primary and specialty health care, special therapies, developmental screening, physical assessments and services to address social determinants that impact the child's development and overall health. Contacts with the child and their caregivers may occur in multiple settings including the health care provider office, community setting, or patient's home, as well as by phone or video conference. CMARC care managers work with other professionals in the Clinical Services Division in the delivery of appropriate services. The care manager assumes responsibility and accountability for the practice of professional care management in a manner consistent with the standards set by the agency. An anticipated hourly salary range for this position is $24.71 to $28.72.
The duties listed below are not all the duties that may be assigned, but are those considered essential for an employee to perform.
* Provides comprehensive victim and family advocacy services, including crisis intervention, trauma education, systematic guidance, and any needed referrals. Completes referrals for trauma-informed therapeutic services. Please note that this can occur during nontraditional hours and on an "on-call" basis. These services can include necessary emergency responses to local hospitals, based on need and an on-call rotation.
* Documents all activities, including family advocacy services/service referrals/follow-up contacts. Prepares information for bi-weekly MDT case review meetings.
* Participates in local, professionally enhancing, and state-mandated trainings. Participates in MDT meetings where specific cases are discussed in a multi-disciplinary setting. Participates in monthly staff meetings to discuss needs, updates, and other relevant topics.

* Master's degree in social work from an accredited school of social work.
* Bachelor's degree in social work from an accredited school of social work with a Child Welfare Collaborative (CPS positions only).
* Bachelor's degree in social work and one year of experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, or treatment planning.
* Four-year degree in the human services field with at least 25 semester hours of approved coursework and two years of experience in a related area.

Summary Job Description:
Provide social work and case management services for members, families and other providers/funding sources to ensure coordination of care for assigned members.
Specific Responsibilities:
Coordinate new admissions to program with Admissions and Program staff
Complete Incident Reports following process indicated by Manager Care Organization and PT's P&P
Participate in the treatment/recovery planning process with program and staff members
Provide documented case management services to members and families including:
Coordination of financial eligibility and benefits
Coordination of family contact and support
Conduct utilization and member care reviews with MCO's and other payers
Oversee Level of Care changes and documented aftercare planning with members
Communicate with both referral services and hospitals to facilitate member return to program in a well-coordinated manner
Consult with families, psychiatrists, therapists for assigned members
Prepare and provide group services at assigned program
Participate in phone coaching/crisis management system which includes:
Orient member and family to phone coaching system
Carry and respond to coaching phone calls on assigned shifts
Attend Team meetings, consultation team and staff meetings
Back-Up Support to Program Director (coverage)
Complete continuing educational requirements as aligned with regulatory compliance

Qualifications:
