How you will change lives As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation. Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas Community Healthcare of Texas is currently recruiting for a Social Worker. POSITION SUMMARY To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patient's needs/wishes with team members, family members, caregivers, and other providers. ESSENTIAL FUNCTIONS Assess the psychosocial needs of the patient and family members Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues Contribute to and execute the Plan of Care with family/patient Record daily documentation of visits and contact in accordance with Policy Provides referrals to community resources Performs in-office/non-clinical administrative duties Perform other duties as assigned POSITION QUALIFICATIONS Bachelors of Social Work with 1 year of applicable experience required; Master's degree preferred Texas State license required Reliable transportation and valid driver's license. Hospice or healthcare experience preferred.
