Job Summary: This Outpatient Physical Therapist opportunity is full time at 40 hours per week, offers full benefits and a convenient schedule Responsible for evaluation, planning, directing and administering physical therapy modalities of treatment as prescribed by a licensed physician. Administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The staff therapist coordinates, delegates and supervises responsibilities assigned to supportive staff (RCNA, PTA, PTLA, aide etc.). The staff therapist participates in operational aspects of the department, maintains performance improvement activities within the department and participates in CQI activities. Provides input in formulating budget, assists in evaluating department performance versus budget controls and takes appropriate action to remain in budget guidelines. Ensures that patient charges are accurate and entered on a timely basis. Participates in all infection control, departmental equipment training and organizational safety programs.
Job Duties/Responsibilities:
Ability to perform an appropriate assessment on all patients as related to the therapy requested and provided and reassessments as per policy. This includes neonate, pediatric, adolescent, geriatric patients and the general adult patient population.
Ability to observe and evaluate treatment effect, recommends change to physician as needed
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate

The LCSW identifies, triages, and manages patients with medical and behavioral health conditions within Barton Community Health Center and Barton Psychiatry & Mental Health offices. As a Thyme Care Oncology Social Worker, you'll be a vital resource for our members and their care network, offering comprehensive psychosocial and emotional support during their cancer journey via phone, email, text, and telemedicine. Primary responsibilities include:

WHAT YOU'LL DO
As a Thyme Care Oncology Social Worker, you'll be a vital resource for our members and their care network, offering comprehensive psychosocial and emotional support during their cancer journey via phone, email, text, and telemedicine. Primary responsibilities include:
Assessment and Counseling: Conduct comprehensive psychosocial assessments to identify patients' and families' needs to address emotional concerns such as anxiety, depression, grief, adjustment, and coping mechanisms related to cancer diagnosis and treatment. Assess for and address financial toxicity, distress, changes in relationships/roles, custody planning, caregiver demands, advance care planning, and spiritual/existential distress. Assess, diagnose, and provide therapeutic interventions to address behavioral health needs.
Crisis intervention: Providing immediate support and intervention in behavioral health crises and sudden changes in prognosis or end-of-life decisions.
Resource linkage: Identifying and connecting patients and families with relevant community resources when there are complex social or behavioral health needs.
Care coordination: Collaborating with the healthcare team, including oncologists, behavioral health providers, nurses, and other specialists, to ensure patients receive coordinated care and address any social or emotional barriers impacting treatment adherence.
Advocacy: Advocating for patients' needs with insurance companies, healthcare providers, and other relevant agencies to access necessary treatment and support services.
Education and support group facilitation: Leading support groups for cancer patients and their families, providing education on cancer-related topics, and facilitating peer-to-peer support.
As a growing company with an evolving care model, responsibilities may shift & additional responsibilities may be required.
This role reports to our Director of Social Work and can be remote or hybrid based in our Nashville or New York City offices. All patient interactions will be virtual via telephone, video, text, or our proprietary virtual care platform.
WHAT YOU'VE DONE
Qualifications: Master's degree in Social Work from a school or program accredited by the Council on Social Work Education (CSWE). Valid and current master's level license in current practice state (i.e., LISW, LCSW).
Certifications: OSW-C or APHSW-C preferred.
