Ensure residents referred as the result of behavioral health concerns receive appropriate treatment and services to correct the assessed problem or attain the highest practicable mental and psychosocial well-being. Ensure residents referred due to behavioral health concerns who display or have been diagnosed with dementia, receive the appropriate behavioral health services to attain or maintain his or her highest practicable physical, mental, and psychosocial well-being. Support Hebrew Home in its provision of medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident. Create an environment of non-pharmacological behavioral strategies to address behavioral health issues related to residents/patients who have been referred. Collaborate with the Care Team in identifying and implementing behavioral modification/intervention strategies with a focus on reducing use of psychotropic medications. SHIFT & STATUS This is a FULL TIME EXEMPT position with a Monday thru Friday schedule. JOB REQUIREMENTS Assists the Resident in the exercise of his/her rights and informs the Resident of any responsibility's incumbent upon him/her in the exercise of those rights. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the Residents cared for. Demonstrates an understanding of the range of care needed based on the age of the Residents cared for.
