The Licensed ClinicalSocialWorker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients and emergency department patients. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual patients, families and at risk populations. ClinicalSocialWorkers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. The functions of the ClinicalSocialWorker includes crisis intervention, patient /family intervention, high risk screening, brief counseling, referring for financial or other identified resource needs, arrange and facilitate family/patient representative meetings with the health care team as needed, assist in post acute placement on complex discharges and engagement of appropriate agencies or community resources when high risk patients are identified. The ClinicalSocialWorker establishes and maintains professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients and families. The position will be required to serve on hospital committees and take part in developing and presenting in-services to Hospital staff.
Job Requirements
At least two years of clinical and/or medical social work experience required.
Should have experience working in NICU or Maternal Health.
Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse, neglect or violence; community or financial resources for underserved or vulnerable populations. SSDI, SDI, and other governmental/county requirements/programs.
CCS required after 90 days
Thorough comprehension of OBRA and Title 22 is required.
Excellent customer service and presentation skills are a must
Strong interpersonal and written communication skills are essential
Demonstrated ability to apply analytical and problem solving skills
Demonstrated ability to manage multiple tasks or projects effectively Ability to work independently as needed with a high degree of detail orientation.

ESSENTIAL JOB FUNCTIONS
Effectively communicates program, administrative, and clinical direction to case management staff. Discusses problem cases with workers and makes suggestions and recommendations. Reviews case records for accuracy, completeness, consistency, and quality of mental health services provided including the application of proper techniques; and ensure compliance with state and federal mandates. Consults with individual case management workers to promote staff development and to suggest solutions to problem cases. ENHANCE CARE MANAGEMENT PROGRAM
Enhanced Care Management (ECM) is a new, statewide Medi-Cal benefit to provide a whole-person approach to care of high-need populations enrolled in Medi-Cal Managed Care. The goal of the ECM program is to effectively manage the medical and psychosocial needs of these members to ensure a well-managed health condition. The Clinical Supervisor will also assist the ECM Director with the clinical and care planning work of a team of Care Managers supporting the holistic care of Managed Care Medi-Cal members enrolled in the ECM Program.
KEY RESPONSIBILITIES
• Provides clinical oversight for ECM, review member risk assessment and care plans.
• Supports in-house training and provides ongoing behavioral health education to ECM staff.
• Conducts case note reviews to ensure quality care management services are provided and documented.

Under Supervision, the MSW provides high-quality direct individual and group therapeutic services while fostering teamwork among staff.
ESSENTIAL FUNCTIONS:

Provide individual and group counseling services and outreach based on the needs of the patients;
In collaboration with patients and the SRC team, evaluate needs of patients and development treatment/service plans accordingly;
Develop effective, trusting relationships with patients, with a focus on facilitating independence, empowerment, and improved physical and mental health;
Develop and maintain complete, accurate, and current patient records in electronic databases and/or patient charts as required by SRC and county standards;
Ensure that all notes are up to date in the SRC system within 48 hours of delivering services;
Provide case updates as requested by supervisor and in SRC team meetings;
Ensure that mental health services are reflective of SRC model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation;
Provide input and evaluation of programming as a participant of SRC's quality improvement program;
Collaborate in the development and provision of clinical trainings;
Provide case updates as requested by supervisor and in SRC team meetings;
Complete Adult Initial Assessment, Discharges, and Progress Notes on time and within industry, DHS, ODR, and SRC standards of practice without exception;
Meet productivity goal of a minimum of 65% of work time in direct service provision;
Assist in providing clinical information and support to the SRC treatment team;
Follow all COVID policies including cleaning, screening, and other protocols
Follow all company and government policies relevant to SRC operations;
Work proactively and cooperatively with co-workers, staff, and management;
Attend staff meetings, trainings, huddles, and other meetings as requested by SRC;
Other duties as assigned.
QUALIFICATIONS:
Masters in field of social services (Social Work, Marriage &Family Therapy and Clinical Psych), with at least 1 years of experience working with marginalized populations
Preferably, experience working with the homeless, substance abuse, mental health/ with dual diagnosis population
Work at a traditional housing facility preferred
Bilingual Spanish/English is preferred but not required
