Assesses the fit and function of the device and the need for adjustments to ensure proper utilization. Develops with the resident and other personnel, as appropriate, a wearing schedule to begin functional use. Educates the resident/family about the proper fit and function of the device and signs of malfunction which would indicate the need for intervention. Resident Goal Setting: Formulates functionally based/objective short and long term goals. Incorporates evaluative findings in establishing time oriented functional and measurable short and long term goals. Respect of Resident's needs: Administers the evaluations in a manner appropriate to the resident's disability, demonstrating respect for the resident's needs. Safety and Body Mechanics: Therapy is provided in a safe environment, utilizing proper body mechanics. Treatment Program: Develops and implements an individualized program of treatment focused on identified resident needs. Focuses on achievement of the established goals. Participates in the ongoing development of the discharge plan that was initiated at the time of initial evaluation. Provides the appropriate assistive/adaptive devices and durable medical equipment to the resident to improve their independence in function. Ensures the proper utilization and clinical appropriateness of the device/equipment and educates the resident/family about its use and safety. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior. Qualifications: Education, Knowledge, Skills and Abilities Required: Graduate of an APTA accredited program or equivalent. Standing, walking and/or continuously moving around. Kneeling, stooping, crouching, crawling or working in other unusual physical positions. Climbing (including ladders, stepping stools, and/or stairs) or balancing. Stretching and/or reaching to retrieve/replace object. Pushing, pulling, lifting and/or carrying heavy objects (20 lbs. or more) without aid of physical devices. Dexterity and eye-hand coordination to perform activities which involve holding, positioning, manipulating, typing (keyboarding or data entry), etc. Visual ability to monitor instruments and computers, distinguish characters (letters, numbers, etc.) recognize colors, observe physical characteristics, etc. Hearing ability to recognize sounds, sound patterns and/or other auditory stimuli. Physical ability to react/respond quickly in emergencies, including life threatening situations. Excellent written and verbal communication skills. Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. Licenses and Certifications Required: Physical Therapist License.
