This individualized, highly detailed plan clarifies needs, charts a course of action and provides the framework to evaluate progress. Services available to residents include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning, recreation activities, etc.), symptom and medication management, community integration and resource development. On duty around the clock staff work closely with each resident to maximize stability and help in the transition to a less restrictive environment. Longer term assistance is available for people with problems preventing them from living outside a group setting. Community Living Apartments The Community Living Apartments Program offers two types of living arrangements: Maple Street and Satellite apartments. Both include support and guidance through individualized service plans created in partnership with the service recipient. Services are a continuation and expansion of those services provided in the Community Residences and carry the ultimate goal of achieving an independent, successful life for each resident. This program serves residents in Columbia and Greene Counties using a three-pronged system of response; Crisis Prevention, Crisis Intervention, and Crisis Follow up. Duties include: Working as the lead of a two-person mobile crisis team, Going out into the community to assess individuals in the midst of a mental health crisis, Deescalating crises, Making a determination as to proper interventions. Intervening in crisis situation over the phone, Networking with various agencies in Columbia and Greene counties Conducting follow-up case management, Maintaining a working knowledge of resources and providing on-call coverage at times. Proper documentation of all work activities is required and essential for this position. Technological competency in databases, Microsoft products, and others is required. Requirements Must be a practitioner possessing a license or a permit from the New York State Education Department as an LMSW, LCSW, LMHC, or RN with at least 2 years' experience. Must have the ability to work independently, have good problem-solving skills and have the ability to work as part of a multi-disciplinary team. Familiarity with Greene and/or Columbia County preferred. Must be of good character and background and display an affinity for working with people with mental illness and/or co-occurring disorders. Must be self-motivated. Must have excellent verbal, written, communication, and interpersonal skills. Have the ability to maintain professional relationships with staff, providers, participants, and the community. Ability to work with a diverse population of urban, suburban and rural cultures and with community members from varying socioeconomic backgrounds. Demonstrate a commitment to Diversity, Equity, and Inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias. Must be able to work regularly outside the office and in the field to engage with clients. Excellent oral and written communication skills.
