    The Family Intervention Specialist (FIS) provides high level administrative support by conducting research, preparing statistical reports, consumer surveys and referral/discharge tracking. The employee will conduct intensive in-home services for families. The employee will utilize approximately 50% of the work week for administrative work and 50% for direct services Enroll in Wheels of Change at time of hire completeing the first four units within seven business days of hire. The first four units must be completed prior to serving clients. Training for each phase of training must be completed prior to providing services required in each phase. Complete all requirements to be certified in the FCT model within 365 days of start date. Participate in team supervision where cases are reviewed, and feedback is provided. Utilize the FCT model to provide Intensive In-Home servides in Diversions and Reunification cases, adhering to and ensuring fidelity to the FCT model. Conducts family and ivdividual sessions as needed in clients' own homes and other settings when convenient for the family, spending an average of four hours per week and a minimum of two weekly contacts with clients in compliance with expected FCT dosage. Conducts initial and on-going assessments that are strength based and individualized to be incorporated in treatment plans. Complete all documentation in a timely manner, including FCT fidelity documents. Provides services unique to each family that promotes the achievement of safety, permanency, and well-being for the children, including evidence-based and trauma-informed in-home services. Implement interventions to ensure any gaps related to the family's basic needs are filled while helping the family also plan for long term sustainability. Provide youth and families with necessary transportation and help them plan for long term transportation needs. Assist family with access to psychiatric services and medication management. Help families develop a support network in the natural environment. Deliver interventions designed to develop skills in both parents and children to problem solve. Promote and supervise the youth and success in school while helping teach parent to advocate for their children's educational needs. Attend child/youth case related meetings. Attend court, prepare youth and families for court and communicate with the case manager and court staff when necessary, in consultation with the family and DCFS. Include the family in individualized treatment plans that are periodically updated to reflect the goals of the family and child and focuses on empowerment and increasing parent accountability. Track referrals Track admission dates and times Track discharge dates and times Assist in Chart audit to ensure compliance and FCT fidelity. Complete FACT tracker, along with staff dosage and FCT fidelity Complete discharge tracker Assist with monthly reports by tracking hotline reports, successes/barriers. Ensure that all assessment transactions and interpersonal exchanges are performed efficiently and in a professional and timely manner, often under adverse conditions. All assessments must be accurate in accordance with state law to provide the record base for the County Collector to collect the appropriate amount of tax for the operation of schools, cities and county organizations. Ensuring each situation is handled according to the county policies and that assessment values are correctly posted on the assessment records. ESSENTIAL DUTIES AND RESPONSIBILITIES Make personal property assessments both in person, by phone, fax, email and by verifying online assessments. Use titles and/or bill of sale, assessing the personal property of individuals and businesses, for the purpose of taxation by phone, online, or in person. This includes vehicles, boats, utility trailers, motorcycles, ATV, campers, and motor homes. Handle large volume of calls, occasionally with disgruntled citizens, with professionalism and tact. Be able to establish control of the conversation, despite high emotions, in person or by phone. Ensure proper documentation of all information needed for assessments is organized for scanning to ensure proper record retention and retrieval. Make proper updates using copies of recorded deeds, court orders, death certificates and other probate related documents. Conduct all liaison activities in a professional manner to ensure maximum cooperation between and among other Saline County offices, other Assessor offices, and other state and local agencies. Must be able to identify school district boundaries to assign the correct millage rate to personal property and real estate assessments. Update personal information on individual and business assessments such as addresses and phone numbers. Must be able to find out if the customer has been previously assessed in another county, moved from out of state, or is a first-time vehicle owner for tax clearance purposes. Verification of each item on the taxpayer's previous year assessment and removing items when necessary, requesting documentation for such. Assist customers with required forms including Commercial Rendition Forms, Homestead Credit Applications, and Exemption Applications. Correct assessments based on state reports that are designed for the purpose of checking for accuracy. Ensure that all activities are professionally and ethically performed in accordance with state law to protect the individual rights of all citizens of Saline County. Successfully complete continuing education classes as required by the Assessor. Perform any other related duties as required or assigned. Responsibilities: Complete the specifically designed trainings and pass the written examination required for certification. Work closely with professional staff for client monitoring and coordination of mental health and functional services. Uses the Master Treatment Plan to develop activities to use with clients approved for QBHP services. May provide on-site and off-site intervention to clients under the supervision of the LC. Identify and provide crisis services as necessary. Makes immediate assessment and responds according to accepted intervention methods and techniques and coordinates other services as appropriate. Participates in staffing, as appropriate, to keep abreast of therapy goals and progress. Ensures that all progress notes relating to client intervention reflect Treatment Plan goals and objectives and current diagnosis are in EMR. Participates in Agency-wide training programs/in-services. Is familiar with the company's policies, procedures, and guidelines including all safety protocols and rules for facility. Reports to work dressed appropriately and complies with established company policies regarding dress code. Ensures confidentiality of client and employee information and responds to internal and external information according to company policy. Assists in overcoming barriers to treatment of clients. Completes tasks necessary to maintain the course of treatment, such as obtaining parent signatures, scheduling, and participating in independent assessments, other assigned tasks. Serves as part of the client's treatment team. Provides professionally appropriate transportation as necessary to accomplish treatment goals. Makes necessary referrals and serves as liaison with other agencies. Provides timely documentation to support services rendered and provides written reports as required by other agencies (i.e. DCFS, legal systems, etc.). Ensures client's attendance to initial medication evaluation and subsequent medication appointments. Supports medical records' staff in securing PCP referral in a timely manner. Schedules supervision and observations as outlined by company policy. Documents all supervision and observations to be reviewed and signed by MHP. Models stability for clients and family with regular sessions. Empower clients to improve day to day functionality, reducing stressors and increasing ability to focus on treatment goals. Encourages client compliance with the established treatment plan. Monitors/reports client behavior in all environments. Attends all required clinic meetings. This position will also use the treatment plan to address and assist with functional needs through the client's treatment. Conduct psychoeducation groups daily and chart for billable services following state and facility guidelines. Responsible for completing drug and nicotine testing for clients as dictated in the client's treatment plan. Communicates with other Team Members to ensure clients are checked in. Other duties as assigned.
