The Licensed Behavioral Health Therapist (LCSW / LCMHC / LMFT)) is responsible for counseling clients; helping them gain access to various resources and assisting clients to confront issues that are negatively affecting their way of living such as poverty, abuse, mental health and substance use.
ESSENTIAL DUTIES:
Performs diagnostic assessments.
Interviews individuals and groups to assess needs, as well as develops appropriate response plans for their clients.
Provides information and referrals for counseling and service acquisition as appropriate to the assessed need.
Provides consultation services.
Initiates treatment interventions; e.g., individual, family and group counseling and crisis intervention.
Helps people in accepting various situations with emphasis on events that can't be changed.
Documents client interaction and referrals.
Collaborates with Primary Care providers.
Performs patient education.
Makes home visits as needed.
Charts and maintains notes in a timely manner.
Participates in data collection and compilation.
Completes all necessary statistical reports and is accountable for accuracy.
Develops and implements policies and procedures.
Protects confidentiality and the abides by HIPPA Laws.

Qualifications
This position is responsible for providing intensive clinical therapeutic services to clients with identified mental health or substance abuse diagnosis.
Responsibilities Include:
Provide clinical services using evidence-based practices in a person centered manner, including admission assessments and comprehensive clinical assessments
Participate in creation of Person Centered Plan (PCP) meetings and amend as required by consumer needs
Participate in service planning, therapeutic intervention, related case support, and documentation in clinical records
