Primary Accountabilities:
Consult and collaborate on a multidisciplinary team to provide integrated on‐site care, recommendations, and feedback to physicians, physician residents, and advanced practice providers. Collaborate with care teams to effectively and efficiently treat and manage patients with chronic emotional and/or health problems. Collaborate with the care team to refer cases to mental health specialists as appropriate. Identify "at risk" patients and develop care plans to promote psychological or physical well‐being. Build care plans that address relapse and morbidity prevention in conditions that tend to recur over time. Collaborate on patient care plans with the care team. Teach patients, families, and staff care, prevention, and treatment enhancement techniques. Attend and participate in meetings and Quality Improvement activities as appropriate. Respond to patient or co‐worker complaints and work toward a positive resolution of any dispute. Work effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect. This includes active and positive participation in continuous process and quality improvement efforts to improve patient care within the organization's care model framework. GENERAL DESCRIPTION
TJJD addresses the multiple and complex needs of the youth committed to the agency as well as their families through a trauma informed multidisciplinary integrated approach that includes implementation of the agency approved Texas Model and a Mental Health Treatment Program (MHTP). The MHTP provides enhanced psychiatric and psychological assistance to youth who have been diagnosed with moderate to severe mental health problems and/or illnesses, which allows the general rehabilitation case managers to focus on risk reduction and protective factor enhancement strategies in order to reduce the risk of reoffending. This collaboration allows for holistic and individualized treatment of youth in need of MHTP services.
Performs entry-level psychological work. Work involves assisting in planning, organizing, coordinating, and monitoring specialized treatment programs. Assists in meeting program goals and objectives; and provides Mental Health Services to youth. Implements and adheres to program standards; and fiscal, personnel, and legal policies.
Works under close supervision, with minimal latitude for the use of initiative and independent judgment. May be required to work in excess of 40 hours per work week.

About the Role
As a Clinician at CCCC, you will provide direct clinical and support services to both resident and non-resident clients. This includes counseling, crisis intervention, support groups, prevention education, and program leadership for children's services. Position Summary
Responsible for providing direct clinical and support services to non-resident clients and/or resident clients, including short-term counseling, support group, crisis intervention, and information/referral. Collaborates in case review meetings with the non-residential and residential client services teams. Serves on SANE on-call on a limited, scheduled basis.
Implementation of agency outreach education in the schools and community, as well as program services for children of agency clients. Outreach and education objectives intended to reduce domestic violence include education about healthy relationships, stalking and dating violence, sexual assault awareness, and prevention. Provides program leadership and oversight to the children's program.
Implement, manage, and facilitate support groups built around trauma-informed care, adult and children.
Actively participate in facility functions with a solution-focused mindset, which is represented through flexibility, collaboration, and a “can do” attitude to ensure organizational success.
Key Responsibility Areas:
Provide individual counseling and advocacy to victims of family violence, sexual assault, and/or incest for both resident and non-resident clients.
Develop Mental Health Assessment Plan, which may include, but is not limited to, safety plans, service goals, measurable outcomes, and time-limited objectives.
Plan and conduct short-term counseling sessions (up to 10 visits) with client/family.
Maintain accurate client records and documents, including timely and complete data entry.
