Determine an individual's appropriateness for care management, medication management or therapy and assign to all that are appropriate. Disposition consumer to appropriate Agency for other mental health services when ineligible for care management. Identify individuals that are in need of MCRT services rather than a routine intake. If appropriate, facilitate crisis action. Assist in resolving any grievance received from a consumer regarding referral. Fill in for Admissions Coordinator as indicated by supervisor. Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review. Contact consumer with decision. Utilize evidence based therapeutic framework, counsel consumers and patients, individually and group sessions, to assist in overcoming dependencies, adjusting to life and making changes. Guide consumers in the development of skills and strategies for dealing with their problems using evidence-based modalities. Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships. Develop and implement treatment plans based on clinical experience and knowledge. Modify treatment activities and approaches as needed in order to comply with changes in consumer's status. Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
