Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans Participate in planning for and the execution of patient discharge experience. Monitor compliance with regulations for orthotics and prosthetics ordering and payment. Make appropriate/timely referrals, including documentation to post discharge providers/physicians. Ensure accuracy of discharge and payor-related information in the patient record. Participate in utilization review process: data collection, trend review, and resolution actions. Participate in case management on-call schedule as needed. Qualifications - License or Certification: Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling). If licensure is required for one's discipline within the state, individual must hold an active license. Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position. CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position. - Minimum Qualifications: For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree. Purpose & Impact: The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency's core values. Essential Functions: Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing. Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child. Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced. Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format. Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN. Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary. Arrange for, attend, and participate in individual case staffing as necessary. Complete all required staffing and application packets. Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary. Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk. Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members. Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective caregivers as needed. Plan and facilitate parental and sibling visits as needed and appropriate. Personal vehicle utilized to transport children as needed. Ensure that all Independent Living functions are completed as required. Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor. The Florida Department of Children and Families (DCF or Department) is the state of Florida's social services agency. The agency oversees services for child safety, fostering, adoption, domestic violence, adult protective services, refugees, homelessness, mental health, substance abuse, childcare providers, human trafficking, and public assistance. The agency's mission is to work in partnership with local communities to protect the vulnerable, promote strong and economically self-sufficient families, and advance personal and family recovery and resiliency. Within DCF, the Substance Abuse and Mental Health (SAMH) Program office is recognized as the single state authority for substance abuse and mental health services. The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders. TO BE CONSIDERED FOR THIS POSITION RESPONSES TO THE QUALIFYING QUESTIONS ARE REQUIRED AND MUST BE VERIFIABLE BASED ON YOUR SUBMITTED APPLICATION. THIS IS NOT A TELEWORK POSITION. The BHC works directly with Child Protective Investigators (CPIs) and dependency case managers to enhance recognition of behavioral health concerns, improve family engagement, and ensure timely access to appropriate treatment and support services. Specific Duties and Responsibilities include: * Consult and collaborating with CPI and dependency case managers to help frontline staff identify behavioral health disorders particularly substance use disorders, enhance family engagement, and improve access to appropriate treatment, recovery support, and community resources. BHC actively builds relationships with investigative staff within co-located offices, providing regular education on behavioral health indicators, the effects of behavioral health conditions on parenting behavior, and engagement strategies. BHCs play a key role in educating and training investigative staff and case managers on the use and benefits of Medication-Assisted Treatment (MAT). * Provides both in-field and office-based consultation during the pre- and post-commencement phases of child protection investigations. This includes participating in joint visits with CPIs, modeling effective family engagement techniques, and offering guidance on best practices for working with families affected by behavioral health issues, particularly opioid misuse. In addition, BHCs assist CPIs in understanding how a parent's behavioral health condition may impact their ability to safely and effectively care for their child, while identifying and recommending appropriate treatment and referral resources within the community to support the family's specific needs. * Community collaboration and care coordination. Partner with behavioral health providers and the Managing Entity (ME) to maintain current knowledge of referral processes and treatment options, helping CPIs connect families to the most appropriate levels of care. * Tracking and documenting all cases where the BHC provides support to child welfare staff, including staffings, joint visits, consultations, brief assessments, and any additional tracking as directed by the department, especially those impacted by substance use disorders. * Participate in legal proceedings, multidisciplinary meetings, and family navigation staffings to support case progression and improve outcomes for children and families. * Offer subject-matter expertise in the identification and care of substance-exposed newborns, including providing consultation on treatment options, supporting care coordination for mothers and infants, and ensuring access to early intervention and family stabilization services. * May be called upon to perform duties beyond those outlined in this description to meet emergent organizational or regional needs. * Work extended or irregular hours, including nights, weekends, and holidays, and potentially operating from outside assigned duty location. * Travel is required throughout the state and may include overnight stays. May also be called upon to assist with disaster recovery efforts and participate in Critical Incident Rapid Response Team (CIRRT) case investigations as needed throughout the state. Emphasis is placed on expertise in addressing opioid use disorders.     The Crisis Center of Tampa Bay brings help, hope and healing to people facing serious life challenges or trauma resulting from sexual assault or abuse, domestic violence, financial distress, substance abuse, medical emergency, suicidal thoughts, emotional or situational problems. The position of Therapist I provides ongoing counseling for individuals who have been victims of trauma and their families. This position provides services under the direct supervision of and reports to the Manager of Clinical Services and in their absence, the Director of Clinical Services. Strategic/Transformational Duties and Responsibilities * To provide help, hope and healing to victims of trauma by providing ongoing counseling. Also assists with raising awareness in the community through events. * Involves staff and other stakeholders in the performance quality improvement (PQI) process and uses data to improve services and outcomes. Transactional/Administrative Duties and Responsibilities * Provide therapy for survivors of trauma, including but not exclusively to, domestic violence survivors, sexually abused juveniles and adult rape victims, adult incest survivors and their families, individually, in group sessions, and/or family sessions. * Provide case management services for clients. * Consult with and make recommendations regarding treatment and treatment needs to Child Welfare and the courts as requested. * Conduct assessment interviews with potential clients. * Document client cases within one week of seeing the client and complete psychosocial histories, treatment plans, other documentation, and diagnosis using the DSM following protocol. * Participate in community awareness and advocacy activities. * Participate in regular clinical supervision, as required by Department of Professional Regulations. * Complete State required outcome measures; receive State Rating Certification as required by funding source. * Complete ACT Service Training as required by FCASV. * Assist in coordination of clinical services as assigned by Manager of Clinical Services. * Represent the Agency at appropriate meetings or task forces. * Assist with supervision of interns. * Assist with training programs related to trauma. * Perform such other duties as may be assigned by the Manager of Clinical Services or Director of Clinical Services.
