ESSENTIAL DUTIES & RESPONSIBILITIES

What You'll Do
As a key member of the interdisciplinary team, you will:

Responsibilities
Demonstrate personable and competent communication skills to develop rapport with clients and referral sources
Present programming information in a thorough and supportive manner to help clients gain access to care
Complete virtual biopsychosocial assessments for a wide range of ages, including children, teens, young adults, and adults with various mental health concerns
Determine appropriateness and fit for virtual intensive outpatient program (IOP) level of care
Construct provisional mental health diagnoses utilizing DSM-V criteria
Document client information in accordance with regulatory standards using electronic medical record
Assign treatment team members and group schedules while informing staff of new admissions
Interface with Admissions, Verification of Benefits, Utilization Review, and Clinical teams in facilitating intake
Collaborate with referral sources: hospitals, treatment centers, psychiatrists, therapists, and other behavioral health providers upon admission
Looking to cover the following shifts to meet the schedules and needs of our clients:
Mornings and Afternoons - Must be available 11am - 7pm MT
Afternoons and Evenings - Must be available 12pm - 8pm MT
Weekends encouraged

Minimum Qualifications
Community Youth Specialist requires a bachelor's degree in social work, psychology, counseling, sociology, criminal justice, juvenile justice, or other closely related field and applicants must have at least two years of work experience providing case management to juvenile offenders. A master's degree in one of the above fields may substitute for one year of the work experience requirement.

Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
U.S. Citizenship is required for this position
Ability to travel up to 75% of the time on a regular basis is required

Nexus-PATH has an opportunity to hire a full-time School Based Therapist who will serve the Fargo, ND Nexus-PATH office! Work with the local school district in order to provide overall management of youth's treatment through active engagement, clinical assessment, treatment, and discharge planning. Therapist will provide clinical insight and work from a trauma informed lens to enhance the milieu and programming. Engage with families through family therapy and treatment planning. Collaborate with the community and providers related to the youth's progress and discharge planning.

Primary responsibilities:
* In coordination with the team, creates and drives a trauma-informed environment, establishing and maintaining a positive treatment culture, and making necessary changes to ensure success
* Collaboration with youth and family to lead and develop to the Nexus Individual and Family Guidance Plans; ensuring staff understand and utilize them in adherence to policy
* Engage in and have active involvement of the Family Collaborative Intake meetings
* Complete Child and Adolescent Needs and Strengths functional assessment in accordance to the TCOM (collaborative) principals
* Through the collaborative effort with family, youth, and providers, develop and monitor the individual treatment plans
* Provide individualized, youth-focused, strength and evidenced-based family and individual therapy models to youth and families. Examples: Trauma Focused Cognitive Behavior Therapy and Eye Movement Desensitization and Reprocessing.
* Attendance and participation in mandatory clinical supervision and team case consultation meetings in accordance with program expectations
* Develop and facilitate milieu and treatment groups that are trauma informed, educational, and treatment focused
* Participate in all youth related treatment team meetings, using family centered language and youth driven principals
* Serve as a clinical treatment team leader to students. This includes but is not limited to modeling and coaching staff on rapport building with clients, trauma-informed practices, and de-escalation techniques on the treatment milieu as needed
* Be available on-call for consultation/treatment direction and/or decisions when appropriate
* Assess readiness for discharge in conjunction with the treatment team. Coordinate and recommend therapeutic services for success after discharge
* Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition
* Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process
* Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional manner according to policy and regulated statutes
* Educate and facilitate trainings for staff on trauma informed practices, clinical diagnosis treatment processes, and other clinically relevant training material
* Participate in team meetings, program clinical team meetings, therapy clinical supervision sessions, and in-service as required
* Ensure confidentiality regarding mental health, medical issues, background treatment and comply with all HIPAA related requirements
* Remains current on all company policies, assigned trainings and/or certification requirements
* All other tasks and duties as assigned
* Recognize and value cultural differences in all aspects of work and service delivery
Required Education and Licensure:
* Master's degree in one of the behavioral sciences, human services, social work, marriage & family therapy, psychology, counseling, or clinically related fields
* Fully licensed in appropriate state as an LMSW, LPCC, LPC, or LCSW
* Valid driver's license required. Must meet state regulating agency and Nexus Home Office driving requirements
* Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
* Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
Preferred Education and Experience:
* Related experience working within the school system
Expected Hours of Work: Days and hours of work are Monday through Friday, with the expectation to work specific schedules and hours as identified and communicated by management. Occasional evening and weekend work may be required as job duties demand.
