Provide orientation to children in care.

MINIMUM QUALIFICATIONS

The Role Itself
Coordinates care with interdisciplinary teams and external providers and updates treatment team on clinical information.
Participates in treatment planning, including psychosocial assessments.
Provides resource information and "warm-referrals" to community providers for engagement.
Completes thorough, timely documentation in compliance with standards.
Demonstrates crisis intervention skills and upholds confidentiality.
Completes the admission process, including verification of benefits and necessary authorizations.
Education:
Bachelor's degree (LSW or RN preferred), or Master's degree in social work or counseling with licensure.
Must be 21 yrs or older.

An opportunity exists to join the clinical team at Seymour: Permanent Part time - 0.8 FTE per fortnight.
The suitable applicant with need to demonstrate strong clinical & assessment skills, ability to work collaboratively with external stakeholders & community partners.
A Community clinician provides a critical role in the active coordination of care for people requiring further follow up & linkages within the community.
The clinician will be required to communicate with referrers & treating teams as required & ensure that service continuity is maintained for consumers, supports & family receiving assistance within mental health services.
